ObjectivesThe purpose of this study was to explore the basic expression law of the Topo Ⅱ alpha and class Ⅲ beta tubulin in breast cancer, and to investigate whether their expressions can predict the response to neoadjuvant chemotherapy with anthracycline or taxane drug respectively or not, additionally to evaluate whether these two molecular markers can be used as sensitive predictors for choosing an individualized chemotherapy regimen in breast cancer.MethodsFrom November2011to January2012,64cases of women patients with primary breast cancer in Xiangya Hospital breast surgery department were enrolled in our study. They randomly received different neoadjuvant chemotherapy regimens by3~4cycles, which including anthracyclines+cyclophosphamide, taxane+cyclophosphamide and anthracycline+taxane+cyclophosphamide. Subsequently, we evaluated the efficacy of chemotherapy regimen. And the expressions of Topo Ⅱ alpha and class Ⅲ beta tubulin in puncture specimens were detected by immunohistochemistry. To investigate whether their expressions can predict the response to neoadjuvant chemotherapy with anthracycline or taxane drug respectively or not, statistical analysis was carried out by SPSS17.0system.Resultsa.The positive expression rate of Topo Ⅱ alpha is59.38%and of class III beta tubulin is64.06%.b. The expressions of Topo Ⅱ alpha had no obvious correlations with clinical stage, the number of axillary lymph node, the status of ER, PgR, HER2and Ki67. The expression of class Ⅲ beta tubulin had no obvious correlations with the number of axillary lymph node, the status of ER, HER2and Ki67, neither. The correlation between clinical stage and class Ⅲ beta tubulin is positive, while between progesterone receptor and class Ⅲ beta tubulin expression is negative.c. The curative effect of chemotherapy regimen with anthracycline is related to Topo Ⅱ alpha expression. Patients with Topo Ⅱ alpha positive can obtain better curative effect by accepting anthracycline. In the patients with HER2overexpression, if Topo Ⅱ alpha is positive, their curative effect of anthracycline is better than those whose Topo Ⅱ alpha is negative.d. There was no correlation between the efficacy of chemotherapy regimen with taxane and class Ⅲ beta tubulin. If breast cancer patients with the co-expression of Topo Ⅱ alpha and class Ⅲ beta tubulin, anthracycline could be a quite sensitive chemotherapy drug. Meanwhile, these patients may have drug resistant to chemotherapy drug taxane. However, when the Topo Ⅱ is positive and class Ⅲ beta tubulin is negative, giving the taxane drug treatment may be obtain better effect than anthracycline.Conclusiona. The curative effect of chemotherapy regimen with anthracycline is related to Topo Ⅱ alpha expression. Patients with Topo Ⅱ alpha positive can obtain better curative effect by accepting anthracycline. Topo Ⅱ alpha may be used as one of the predictors for formulating individualized chemotherapy regimens in breast cancer patients.b. Whether class Ⅲ beta tubulin could be a prediction marker for formulating sensitive and effective individualized chemotherapy regimens in breast cancer patients still needs more samples and further studies.
