—Objective:To compare the clinical efficacy of the artificial joint replacement and PFNA fixed two surgical methods in elderly patients with intertrochanteric fracture, and look forward reasonable recommendations for treatment, to provide guidance for clinical work.Methods:43cases of patients over the age of60intertrochanteric fractures were treated between June2007to September2011according to the surgical approach is divided into A, B groups. A row hemiarthroplasty group B line anti-rotation of proximal femoral nail fixation (PFNA) are observed in patients with surgical time; follow-up outpatient review, telephone and letters, bed time, postoperative complications recovery, according to the Harris hip score were evaluated, the recovery is divided into excellent, good, poor three levels, and calculate each group’s excellent rate.Results:The two sets of phase, A, operative time shorter than the B group (P<0.001); However, the amount of bleeding in group A than group B (P<0.001) and group B in bed time was significantly longer in group A (P<0.001). Group A2cases of complications, and1cases after4days of acute coronary syndrome, and one cases of wound fat liquefaction. Group B, complications, delayed healing of fractures. The two last follow-up scores did not show significant differences (P>0.05).Conclusion:The two methods have their advantages and disadvantages; for the elderly of intertrochanteric fracture if the fracture stability, and generally in good condition, good bone conditions can choose PFNA fixation system, rather than the unstable fracture associated with complex internal diseases, bone conditions poor should be considered artificial joint replacement; Select a reasonable surgical approach, can achieve satisfactory results. |
