Clinical Value Of GP73in Diagnosing Hepatocellular Carcinoma And Intrahepatic Cholangiocarcinoma

ObjectiveTO observe the expression of Golgi protein73(GP73) in Hepatocellularcarcinoma (HCC)、intrahepatic cholangiocarcinoma (ICC) and the control group；the positive percentage of serum GP73in AFP-negative HCC； thesensitivity,specificity and accuracy of serum GP73in diagnosing HCC and ICC. Toanalyse the influence of infection hepatitis B virus to the expression of GP73. TOelementary discuss the clinical value of GP73in diagnosing HCC and ICC.MethodsThere is a dataset of60HCC cases and50ICC cases during the period of Sept.,2010to Sept.,2011collected from the second affiliated hospital of Nanchanguniversity, the first affiliated hospital of Nanchang university and The People’sHospital in Jiangxi Province; and30healthy physical examinees’ serum specimencases in Sept.,2011collected from the second affiliated hospital of Nanchanguniversity, with140cases in total.Three groups were divided into the HCC group,the ICC group and the healthy control group.For HCC group and ICC group, there is56patients and31patients with Hepatitis B virus (HBV) background respectively.Patients of the two groups have all had primary surgical treatment and postoperativepathological examination later. Among all60HCC patients, there are37males and23females, with the age from23-72years old and the mean age of56years old.Among all50ICC patients, there are22males and28females, with the age from45-68years old and the mean age of59years old. Among30healthy testing persons,there are both15cases of males and females, with the age from20-60years old andthe mean age of45years old. Before operation, three groups of HCC, ICC andhealthy testing persons have been drawn of2ml fasting venous blood, which iscentrifuged to obtain blood serum, and the serum specimen is placed under-20℃environment for preservation and backup. The ELISA quantitative measurement isadopted to detect the serum GP73and human alpha-feto protein (AFP) level of allgroups.Results 1. Serum GP73protein could been detected in the serum of all groups，but thelevels were the highest in HCC. Serum GP73levels were203.5±136.3ng/ml inHCC．The Hcc ROC curve for GP73was0.82，with a sensitivity of78.3%，specificity of93.3%，accuracy of83.3%,and an optimal cutoff point of87.5ng/ml.While the ROC for AFP was O．65，with a sensitivity of61．7％，specificity of86.7％，accuracy of70%, and a cutoff of20．97ng／m1．GP73had a bettersensitivity， accuracy and AUROC compared to AFP（P<0.05）．The specificity hasno significant difference（P>0.05）. Furthermore, the positive percentage of serumGP73is69.6in AFP-negative HCC.2Serum GP73levels were137.9±101.9ng/ml in ICC．The Icc ROC curve forGP73was0.71，with a sensitivity of64.0%，specificity of90.0%，accuracy of73.8%， and an optimal cutoff point of83.5ng/ml.3. Serum GP73levels were（198.5±126.3）ng/ml in HBsAg-positive PHC，the levels were（193.5±118.3）ng/ml in negative.There are60GP73-positive in86HBsAg-positive PHC and19GP73-positive in24HBsAg-negtive. There is noinfluence of infection hepatitis B virus to the expression of GP73（P>0.05）.4. The Hcc ROC curve for GP73was0.82，with a sensitivity of78.3%，specificity of93.3%，accuracy of83.3%. The Icc ROC curve for GP73was0.71，with a sensitivity of64.0%，specificity of90.0%，accuracy of73.8%.
