ObjectiveTo investigate the curative effects of strengthening the spleen and replenishing qi massage on gastroesophageal reflux in premature infants.And to explore the effect of strengthening the spleen and replenishing qi massage on serum gastrin(GAS) and plasma motilin(MOT) in premature infants with gastroesophageal reflux. With a view to improve the clinical symptoms of spleen deficiency in preterm infants with gastroesophageal reflux, and evaluate the safety of strengthening the spleen and replenishing qi massage.MethodsAccording to the case study of prospective randomized controlled trial design of case study methods, were included in January2011-December2011in Guangzhou University of Traditional Chinese Medicine subsidiary maternity Children’s Hospital Neonatal Intensive Care Center,64cases of premature infants with gastroesophageal reflux, which were randomly divided into experimental group and control group. Control group infants according to management guidelines for premature infants with conventional treatmen, during treatment of gastroesophageal reflux, the control group were given10minutes of non-nutritive sucking before each feeding, raise the head and shoulder30°after feeding. When necessary, given domperidone suspl0.4ml/kg by peroral administration or nasal feeding once in every8hours a day,30minutes before feeding. If the effect is not satisfactory, given the control group intravenous erythromycin5mg/kg for once a day. The course of treatment lasted from3to5days. And the treatment group in the conventional management of premature infants at the same time, to oral strengthening the spleen and repleni shing qi massage,10minutes/time,2times a day,7days for a course. The clinical symptoms and24-hour esophageal pH value were observed before and after treatment. Detected the serum gastrin and plasma motilin levels of two groups before and after treatment. The gastrin and motilin in blood were detected by radioimmunoassay(RIA). Compared the result with statistical analysis. Evaluated the safety of strengthening the spleen and replenishing q1massage.Results1.Preterm infants with gastroesophageal reflux by vomiting, apnea as the main performance. Some patients had electrolyte imbalance, anemia, abdominal distension, cyanosis, aspiration pneumonia and other complications. The symptoms of patients who in experimental group was Improved obviously (P<0.05).2. By esophageal24-hour pH test, the total time of pH<4reflux, the times of pH<4lasts longer than5minutes, and the longest reflux duration in experimental group were significantly reduced than in control group after the treatment (P<0.05).3. The levels of serum gastrin and plasma motilin in two groups were significantly increased after the treatment (P<0.01). And the levels of serum gastrin and plasma motilin in experimental group were significantly higher than those in control group (P<0.05).4. The vital signs of patients with no obvious abnormalities before and after treatment.No one skin damage and faint during massage.ConclusionStrengthening the spleen and replenishing qi massage can significantly improve the clinical symptoms, reduce reflux rate of premature infants with gastroesophageal reflux. And increased the level of the premature infants’serum gastrin and plasma motilin, improved spleen and stomach function. Strengthening the spleen and replenishing qi massage is safe and reliable. |
