Objective: To investigate the expression and mechanism of IL-18/IL-18BP in thepatients’ serum and peripheral blood mononuclear cells with HBV-associatedsub-acute-on-chronic liver failure.Methods: Extracted monocytes in patients of HBV-associated sub-acute-on-chronicliver failure (ACLF),chronic hepatitis B (CHB) and healthy person. we usedImmunohistochemical assay to detected expression of IL-18/IL-18BP in monocytes.IL-18/IL-18BP and IFN-γ in peripheral blood was quantitatively detected by ELISA. Thepercentage of Th1and Th2cells Was detected by Flow Cytometry(FCM).Results:1. The expression of IL-18in the control group, CHB group andHBV-associated sub-acute-on-chronic live failure group were (0.85±0.67),(2.96±1.37),(6.40±2.06) sub, It was significantly increase in the HBV-associatedsub-acute-on-chronic live failure group and the CHB group when compared to normalgroup. The expression of IL-18BP in these three groups were (0.90±0.79)、(3.33±1.63)、(4.28±1.67)sub, It was significantly increase in the HBV-associated sub-acute-on-chroniclive failure group and the CHB group when compared to normal group (P <0.01). but nosignificant difference between the two groups (P>0.05). The expression of IL-18inperipheral blood serum in normal control group, CHB, group, HBV-ACLF were (37.58±26.06),(77.46±63.13),(232.16±227.07) pg/ml. It was significantly increase in theHBV-associated sub-acute-on-chronic live failure group and the CHB group whencompared to normal group (P <0.01). The expression of IL-18BP in peripheral bloodserum in normal control group, CHB, group, HBV-ACLF group were (5.74±0.52),(8.49± 4.52),(8.92±3.43)ng/ml, It was significantly increase in the HBV-associatedsub-acute-on-chronic live failure group and the CHB group when compared to normalgroup (P <0.01), but it was no significant difference between the two groups (P>0.05).The expression of IFN-γ in peripheral blood serum in normal control group, CHB groupand HBV-ACLF group were (33.18±39.45),(70.68±43.67),(173.59±90.92) pg/ml.Compared with the CHB group, the level of IFN-γ was significant increase in theHBV-ACLF group.2. The percentage of Th1cells was significantly increase in theHBV-associated sub-acute-on-chronic live failure group and the CHB group whencompared to normal group (P <0.01). The percentage of Th2cells was significantlyincrease in the HBV-associated sub-acute-on-chronic live failure group and the CHB groupwhen compared to normal group, but it was no significant difference between the twogroups (P>0.05).Conclusion:1.Interleukin-18in monocytes in the patients of HBV-associatedsub-acute-on-chronic liver failure and CHB was increased compared with normal,suggesting that it might be involved in host immune response of hepatitis B virus. IL-18was related to the severity of the disease.2. The percentage of Th1cells was significantlyincrease in the HBV-associated sub-acute-on-chronic live failure group and the CHB groupwhen compared to normal group. The percentage of Th2cells was significantly increase inthe HBV-associated sub-acute-on-chronic live failure group and the CHB group whencompared to normal group, but it was no significant difference between the twogroups.3.the relative level of IL-18and IL-18BPa may be associated with the outcome andprognosis of the disease.
