| Objective To investigate the tolerance and outcome of operation of operativetreatmen and nonoperative treatment for hip fracture in elderly;and study of the influencingfactors on outcome of operation, providing a correct guidance for treating.Methods A retrospective study was done in183(79males and104females) caseswith hip fracture over the age of65who underwent treatment from January2005toJanuary2011in the fourth affiliated hospital of Soochow University. These all cases wereclassified into Surgical group and Non-Surgical group according to the treatmentmethods,then a comparison was made between the two groups in tolerance and curativeeffect. The factors were collected might affect the hip function, including age, gender,body mass index, singh index, traction, basic disease, opportunity of operation, kind ofsurgery and early postoperative complications, treatment of osteoporosis, taking steroidtablets, those were analyzed by single factor analysis and the parameters with P<0.05wereanalyzed by Logistic regression.Results (1) Surgical And Non-Surgical:there was statistical difference in tolerance,curative effect between Surgical group and Non-Surgical group (P<0.05).(2)Age:there was statistical difference between above86years group and othergroup(P<0.05)(3)Gender:there was not statistical difference between male group and womengroup(P>0.05).(4) Body Mass Index:there was not statistical difference between above22.9groupand below22.9group(P>0.05). (5) Singh Index:there was statistical difference between above3level group andbelow3level group(P<0.05).(6) Traction:there was not statistical difference between no traction group andtraction group(P>0.05).(7) Basic Disease:there was statistical difference between With more than three basicdiseases group and other group(P<0.05).(8) Opportunity Of Operation:there was statistical difference between surgery withina week group and surgery more than a week group(P<0.05).(9) Kind Of Surgery:there was statistical difference between joint replacementgroup and internal fixation group(P<0.05).(10) Early Postoperative Complications:there was statistical difference between nopostoperative complications group and postoperative complications group(P<0.05).(11)Treatment Of Osteoporosis:there was statistical difference between treatment ofosteoporosis group and without treatment of osteoporosis group(P<0.05).(12) Taking Steroid Tablets:there was statistical difference between taking steroidtablets group and without taking steroid tablets group(P<0.05).(13) Logistic regression also showed that the the ageã€basic diseaseã€opportunity ofoperation〠early postoperative complications and treatment of osteoporosis wereindependent risk factors affecting hip function after operation.Conclusion1Operation is necessary for hip function in the elderly,as long as thetolerance comprehensive evaluation suggesting that its can tolerate the operationpossiblely.2Hip function was related to the ageã€basic diseaseã€opportunity of operationã€earlypostoperative complications and treatment of osteoporosis after operation. |
