Objective:To evaluate the value of peripheral blood microRNA-23b(miRNA-23b)as the identification molecular marker of gastric cancer and non-gastric cancer lesions (pre-gastric cancer and healthy controls), to verify the positive rate and negative rate of peripheral blood miRNA-23b in the diagnosis of gastric cancer,try to figure out the potential role of miRNA-23b as a new gastric cancer molecular marker applied for clinical screening.Methods:98patients were enrolled for our study peripheral blood specimens and clinicopathological data of the patients were gathered.we used the mirVana PARIS Kit to extract the serum mi RNA and All-in-One miRNA qRT-PCR to detect the expression level of miRNA-23b in the serum of peripheral blood specimens.According to the positive standard of expression level of miRNA-23b in the serum(determined by preliminary results of our laboratory), we discriminate the positive or negative results of miRNA-23b in the serum of the98patients,then according to the clinicopathological data of the patients,we calculated the positive rate and negative rate of peripheral blood miRNA-23b in the diagnosis of gastric cancer.Besides,we analyzed the correlation between the the expression level of peripheral blood miRNA-23b with clinical and pathological data of the gastric cancer patients.Results:1)98studied cases included45gastric cancer patients,29pre-gastric cancer patients (including four cases of low grade squamous intraepithelial neoplasia and25cases of chronic atrophic gastritis),24cases of normal (healthy controls), mean age (53.2±13.8) years,66cases of male and32cases of female.2) The average expression level of miRNA-23b in gastric cancer group (-2.597±3.448)(Log2ΔCt) was significantly higher than that in healthy control group (1.680±2.229) and that in pre-gastric cancer group(0.443±2.198)(p<0.05). Also the average expression level of miRNA-23b in pre-gastric cancer group was significantly higher than that in healthy control group(p<0.05);3) In45cases of gastric cancer patients,the positive rate of miRNA-23b expression was68.9%; in29cases of pre-gastric cancer patients,the positive rate of mi RNA-23b expression was31.0%; and in24healthy controls, the positive rate was8.3%, the positive rate and negative rate of serum miRNA-23b to distinguish between gastric cancer and non-gastric cancer (including pre-gastric cancer and healthy controls) were68.9%and79.2%respectively.The positive predictive value was73.8%, negative predictive value was75.0%.4) For patients with gastric cancer, the expression level of peripheral blood mi RNA-23b was not correlated with age, gender, type of tumor differentiation, tumor invasion and metastasis (P>0.05).Conclusion:Peripheral blood miRNA-23b may can be as a molecular diagnostic marker for gastric cancer,but it still needs further study to expand the sample size before used for clinical screening.The mechanism of the action of miRNA-23b in the development of gastric cancer needs to be explored.
