Subject: Comparison of ventilatory efficacy by laryngeal mask airway(LMA) or endotracheal tube on the neonatus with different ventilatory mode.Objective: The relationship of peak inspiratory pressure (PIP) and tidalvolume (TV), the effect of different respiratory rates to the PETCO2werecompared using laryngeal mask airway or endotracheal tube on the neonatuswho underwent abdominal operation with pressure controlled ventilation (PCV)and volume controlled ventilation (VCV).Methods:Fifty neonatus undergoing abdominal operation (since he wasborn24hours to28days), the score of Apgar>7and the weight>2.5kgafter he was born, were enrolled. After induced by sevoflurane, insertion of theLMA (L group) or endotracheal tube (E group) was randomly, and thenmonitored the PETCO2and the vital sign. Each neonate was randomly receivedLMA or endotracheal tube with different ventilatory mode, the rate35countsper minute, the rate of inspiration and expiration1:2, the PEEP5cmH2O. Thepressure of inspiration were signed16cmH2O,18cmH2O and20cmH2O, at thesame time, monitored the range of the PETCO2. More than15min afterventilation, record the VT, PETCO2ã€HRã€BP. All values were expressed as themean value after measuring there times.Results:(1) Under the PCV, with the14~18cmH2O PIP, there were notsignificant differences in the VT and PETCO2between the L group and the Egroup (p>0.05), but with the20cmH2O PIP, there was difference of VT betweentwo groups. There was no difference of PETCO2with the different respiratoryrates between two groups (p>0.05).(2) Under the VCV, there were not significant differences in the PIP and the PETCO2with the same VT (p>0.05)between the two groups.(3)Under the PCV and VCV, with14~20cmH2O PIP,there were not significant differences in the PIP and the PETCO2by Using theLMA (p>0.05).Conclusion: During the general anesthesia on the neonatus, under thePCV, with14~18cmH2O PIP, using the LMA and Endotracheal tube canensure the efficacy of the ventilation and maintain the normal PETCO2. Underthe PCV, with14~20cmH2O PIP, the VT was significant higher in the E groupthan in the L group. |
