IntroductionSevere Head Injury (SHI) defined by general fracture of skull, contusion and laceration of brain, brain stem injury, intracranial hematoma, coma for more than6hours, or those coma for more than6h after consciousness deterioration in24h of injury. The condition score3to8according to Glasgow score. The wound was not foresee, and different in position, style and degree of wound, in addition to, the structure of brain is special, so it is complex and serious of head injury.325cases severe head injury with Glasgow score below8were treated, remedy experience was summarized in favor of decreasing the mutilation rate and increasing the survival rate.ObjectiveIn order to improve survival rate and cure rate of severe head injury, treatment experience was summarized.MethodsThe diagnosis and treatment of325patients with severe head injury and Glasgow scores of3to8were retrospectively studied and summarized. ResultsIn the325cases with severe head injury, including of196cases treated with surgery (60.3%) and129cases exposed to expectant treatment (39.7%),217patients survived (66.8%), and108patients dead(33.2%). The therapeutic effect of the217survivor comprised:(1)35plant man(10.8%),(2)41patients (12.7%) who were severe disability, with awake mind, and requiring nursing,(3)52patients (16.0%) who were secondary disability, with self-care ability,(4)79patients(24.3%) with good recovery and completely independent living and learning.ConclusionDecreased mutilation rate and death rate, increase the survival rate and curing rate, there are several effective therapeutic methods:1. Closely observing and detecting to vital signs, pupil and consciousness of SHI patients in the early stage;2. Clearance of intracranial haematoma, treatment to contusion and laceration of brain, decompressing with craniectomy in time, and decompressing inner pressure, repairing depressed cranial fracture in necessary;3. Effective releasing of cranial pressure and controlling blood pressure, ensuring smooth respiration, and supporting the circulation;4. Diagnosis and treatment of the primary brain injury and complication in prophase is necessary, supporting nutrition and preventing of complication are needed in the later period, and5. Treatment with hyperbaric oxygen, acupuncture and rehabilitation take effect when the patient’s condition is stable.
