Comparative Study About Renal Cell Carcinoma Retain Nephron-sparing Surgery Laparoscopic And Open Surgical Procedure

Background: Many scholars consider LNSS have less invasive, shorteroperative time, intraoperative blood loss, shorter hospital stay than ONSS.LNSS surgery as a new development, need more time to learn, intraoperativeischemia time, intraoperative and postoperative complications of highincidence.So it is necessary to do a comparison to the two mehods betweenlaparoscopic nephron-sparing surgery and open nephron-sparing surgery, andfurther investigate the therapeutic effect of the two surgical procedures, toprovide further enlightenment on clinical work.Objective: In this study, we compared many aspects the two groups ofLNSS and ONSS and acquired corresponding results, to further explicate theadvantages and disadvantages of both meothds in therapying kidney cancer, toprovide a reference for clinical doctors.Methods: Retrospective analysis of39cases of renal cell carcinomapatients experienced with nephron-sparing surgery,in urinary Treatment Centerof the First Hospital of Jilin University from January1,2009to December2011,30June, including16cases with open nephron-sparing surgery and23caseswith laparoscopic nephron-sparing surgery,to compare the clinical effect of thetwo groups of surgical procedures.Results: The span time of drainage tube inserted is no significantdifference between ONSS group and LNSS group(P=0.09),The LNSSgroup,compared ONSS group,has less operative time,less boold loss,shorterhospitalization and recovery of bowel function time,however The LNSSgroup has more hot ischemic time and more cost than ONSS group,the twogroups have signifiacnt differenec(P=0.00),Two groups of kidney function in the preoperative, postoperative1h, after2days have no significant difference(P>0.05).Conclusion: Despite the LNSS learning,performed more difficultly,need more time,LNSS using minimally invasive techniques on the basis of theONSS retain nephron-sparing,reduce the burden of the unphological changekidney,whatmore shorter operative time,less blood less,shorter intraoperativeh-ospitalized,faster recovery of bowel function et al.LNSS is a safe,effectiveandminimally invasive surgery,under appropriate indications,limitations of less4cm kidney carcinoma can choose laparoscopic nephron-sparing surgery,weconsider that can reduce renal vascular clamping and intraoperativehemostasis time by the clinical experience accumination and explotation ofsurgeons,laparoscopic surgery operation further inprove and the developmentof new equipment.The study needs further and variety randomized controlledresarch.
