Purpose:To assess the feasibility, safety and efficacy of endoscopic extraperitoneal radical prostatectomy (EERPE) with bladder neck preservation (BNP) and specially evaluate the influence on postoperative early continence.Materials and Methods:preoperative data, operative time, blood lose, pathological staging, margin status and early continence status from12BNP and15standard technique(ST) during EERPE were retrospectively assessed. Number of daily pads usage and Stamey Grade were both used to measure urinary incontinence.0~1pad/day was defined as urinary continence,2~3pads/day was defined as mild incontinence,>3pads/day was defined as incontinence.Results:Both groups were similar for age, body mass index, prostate volume, prostate-specific antigen and Gleason score. There was no significant difference in operative time, blood loss, catheter removal time, postoperative hospital stay, postoperative complications or positive margin status.Stamey first grade in BNP in first day,1week,1month and3months after catheter removal were8.3%(1),33.3%(4),50%(6),66.7%(8). The percentage of0-1pad/day patients were25%(3),50%(6),66.7%(8),91.7%(11), respectively. Stamey first grade in ST in first day,1week,1month and3months after catheter removal were0(0),6.7%(1),13.3%(2),26.7%(4). The percentage of0-1pad/day patients were6.7%(1),13.3%(2),20%(3),46.7%(7), respectively. There were significant differences in postoperative early continence between BNP and ST groups.Conclusions:Endoscopic extraperitoneal radical prostatectomy with bladder neck preservation was feasible, safe and efficacious, and proved to have a positive impact on postoperative early continence.
