Background and objective.Esophageal cancer is one of the most common malignant tumors of the digestive system which have become a serious threat to human health.Each year about30million new patients are suffering from esophageal cancer. China is one of the high incidence countries of esophageal cancer in worldwhile, the new cases of the esophageal cancer in China accounts for more than half of the toles of the world every yeat.There is an obvious regional distribution in china,in some places the incidence can reach to100/100000, especially for male which ranks the fifth cancer incidence and mortality. Most of the patients are diagnosed with regional lymph nodes and/or distant metastasis and lost the surgical opportunity. In recent years, despite the diagnosis and treatment technology for esophageal cancer have progressed, its overall5-year survival rate is still hovering around10%. Lymph node metastasis is one of the main factors for cancer recurrence and metastasis, and significantly affects the prognosis and survival rate. The mechanism of lymphatic metastasis as well as its relationships with lymphangiogenesis and lymphatic metastasis is still a hot issue in recent years. CCR7can interact with VEGF-C in the generation of lymphatic and lymph node metastasis. They both influence the formation of tumor lymphangiogenesis and promote lymphatic metastasis. Currently, there are no literature about CCR7and VEGF-C combinding with Podoplanin in the lymph node metastasis in the esophageal squamous cell carcinoma. We use the immunohistochemical method to explore the relationship between the expression of CCR7、VEGF-C as well as Podoplanin with the lymphatic generation as well as lymph node metastasis in esophageal squamous cell carcinoma, in order to find a new indicator to predict the lymph node metastasis of the esophageal squamous cell carcinoma and a new target therapy through interfering lymph node metastasis pathway.Materials and methodsIn this study,40cases were collected to detect the expression of CCR7VEGF-C and Podoplanin in the esophageal squamous cell carcinoma tissues and the Corresponding normal esophageal tissues using immunohistochemical S-P method and count MLVD value. All Statistical data are processed by statistical software SPSS17.0package.Result1The difference of the expression of CCR7, VEGF-C and Podoplanin between esophageal squamous cell carcinoma and the Corresponding normal esophageal tissue is statistically significant.2The expression of VEGF-C、CCR7and Podoplanin in the esophageal squamous cell carcinoma tissue is unrelated with the age, gender, degree of differentiation, tumor size, but has a close relationship with clinical pathological stage and lymph node metastasis.3The MLVD value marked by Podoplanin in the esophageal squamous cell carcinoma is significantly higher in advanced esophageal squamous cell carcinoma than the earlier, and has a higher value in the group with lymph node metastasis than those without lymph node metastasis, The MLVD value is also significantly higher in the peripheral tissues of esophageal squamous cell carcinoma than center The differences are statistically significant. 4The expression of CCR7, VEGF-C in esophageal squamous cell carcinoma have positive correlation.5They are positively correlated with the expression of CCR7, VEGF-C and the MLVD value whitch marked by Podoplanin in esophageal squamous cell carcinoma. The MLVD is significantly higher in the positive expression group of CCR7and VEGF-C. The MLVD value shows an increasing trend with the expression ofCCR7and VEGF-C.Conclusion1The relationship of the expression of CCR7、VEGF-C、Podoplanin with the clinicopathological features suggests that the up-regulation of VEGF-C, CCR7and Podoplanin protein is important in the process of esophageal squamous cell carcinoma lymph node metastasis, whitch may enhance the invasion and metastasis of esophageal cancer.2The expression of CCR7, VEGF-C and the MLVD value marked by Podoplanin are positively correlated with esophageal squamous cell carcinoma, which suggest that CCR7and VEGF-C may promote the formation of tumor lymphangiogenesis in esophageal squamous cell, thus contributing to tumor metastasis through the lymphatic channels.3The detection of VEGF-C, of CCR7and Podoplanin can be used as an objective indicator to determine the invasion and metastasis of the esophageal squamous cell carcinoma and more accurate predictor for the lymph node metastasis, which can give guidance to the treatment of the esophageal squamous cell carcinoma and may provide a theoretical basis and clinical evidence by inhibiting the expression of CCR7, VEGF-C though the reducing lymph node metastasis and lymphangiogenesis.4The Correlation of the MLVD value marked by Podoplanin with the clinicopathological features in the esophageal squamous cell carcinoma suggest that the MLVD value can be used as one of the indicators reflecting the malignant biological behavior of esophageal squamous cell carcinoma.
