Objective:To study the relationship between genetic polymorphisms of Tumor Necrosis Factor(TNF-α-308, TNF-α-238),external exposure factors and the phenomenon of Primary liver cancer family-clustering in Guangxi.Methods:Blood samples collected from124cases of PLC high incidence families and129cases of control families, relevant external exposure factors of PLC were collected by the epidemiological questionnaires in the high risk area of Guangxi, at the same time10ml peripheral blood (EDTA anticoagulant and non-anticoagulant each5ml) were collected, the infection rate of HB V in serum of non-anticoagulant were tested and the serum of EDTA anticoagulant were used to test the genotypes and alleles of TNF-α-308and TNF-α-238. Distribution frequency differences of the genotypes and alleles of TNF-α-308and TNF-α-238between PLC high incidence families and control families in Guangxi was tested by Chi-square test. The non-condition logistic regression were used to analysis the major high risk factors of the familial susceptibility to PLC, then analyzed the relationship of gene-external exposure factors interaction. Results:1、There were no statistically differences in the general demographic characteristics(age, sex, marriage and so on) between cases and controls, respectively(P>0.05).2、The polymorphisms of TNF-a-308and TNF-a-238of control families were in accordance with the Hardy-Weinberg Equilibrium among the controls (P>0.05).3、Among the high risk family group and control group,37.1%and11.6%respectively were HBsAg positive in serum, the difference was significant(x2=22.414, P<0.001), OR (95%CI) was4.482(2.340-8.586).4、The TNF-a-308G/A genotype frequencies, in high incidence of family group and the control family group, were22.6%and11.6%respectively. The difference were significant between two groups(x2=5.376, P=0.020).The A alleles frequencies, in two groups, were11.3%and5.8%respectively. The difference were significant between two groups (x2=4.877, P=0.024). As for the TNF-a-238genotype frequencies, they were5.6%and3.1%respectively. The difference were no significant between two groups(x2=0.984, P=0.321). The A alleles frequencies, in two groups, were2.8%and1.6%respectively. The difference were no significant between two groups(x2=0.962, P=0.335).5、Application of non-conditional logistic regression model of genotype of TNF-α-308and TNF-α-238, HBV infection, corn intake, water type, smoking history, drinking history and analysis found:the major risk factors of the PLC family-clustering in Guangxi is corn-based food, infection of HBV, carrying genotype of TNF-a-308G/A.6、To analysis the interaction that the TNF-a-308gene associated with HBV infection and the staple maize respectively in Primary liver cancer. It showed that TNF-α-308gene combined with HBV infection might no synergistic action on the susceptibility of family-clustering of the PLC in Guangxi (P=0.290); TNF-α-308gene combined with corn-based food might no synergistic action on the susceptibility of family-clustering of the PLC in Guangxi (P=0.068).Conclusion:The polymorphism of TNF-α-308gene and the PLC family-clustering in Guangxi may be relevant. The polymorphism of TNF-α-238gene may be not associated with family-clustering of the PLC in Guangxi. The major external exposure factors were the faction of HBV and corn-based food. On the statistical significance, there may be no interaction that TNF-α-308gene associ-ated with the faction of HBV and corn-based food separately in the PLC family-clustering, which need more interactive cases to proof.
