Objective To study the expression of p53, ki-67in the tissues of hepatocellular carcinoma (HCC) and the relationships between them and clinical pathology and the prognosis of patients with primary HCC after R0resection.Methods (1)The expressions of p53ki-67in HCC tissues of290patients,81cases with adjacent non-cancerous tissues and79cases with cirrhotic tissues were examined by tissue microarray and immunohistochemistry PV-6000methods.(2) The relationships between their expressions and clinical pathological factors, short term recurrence (within6months after resection) and the disease-free survival (DFS) and overall survival (OS) after resection were analyzed.Results (1) The expression rate of p53and ki-67protein in HCC tissues was26.2%and71.0%, there were not expressed in adjacent non-cancerous tissues and cirrhotic tissues (χ2=50.452,209.610, P=0.000,0.000). A significant correlation was found between the expression of p53and vascular invasion and non-envelope of tumor (χ2=9.795,4.620; P=0.002,0.032). The expression of ki-67correlated with non-envelope of tumor and poor differentiation (χ2=7.515,5.607; P=0.006,0.018).(2) The median DFS in p53position group and negative group was15and30months (P=0.020), in ki-67position and negative group was20and50months (P=0.007), p53position+ki-67position group was14months (other patients was30months, P=0.007), and p53negative+ki-67negative group was45months (other patients was20months, P=0.006), Cox regression analysis showed that the ki-67expression was an independent factors for DFS in these proteins.(3) The median OS in p53position group and negative group was34and64months (P=0.003), in ki-67position and negative group was47and90months (P=0.000), p53position+ki-67position group was25months (other patients was64months, P=0.000), and p53negative+ki-67negative group was90months (other patients was48months, P=0.001), Cox regression analysis showed that the ki-67expression and p53position+ki-67position expression were the independent factors for OS in these proteins.(3) In190patients with HCC recurrence after resection, the positive expression of p53and ki-67was higher more than negative expression (χ2=7.499,4.540, P=0.006,0.033) in the short-term recurrence group, p53position+ki-67position group was more than other patients ((χ2=7.241, P=0.007) and p53negative+ki-67negative group was less than other patients (χ2=5.291, P=0.021). Cox regression analysis showed that the expression of p53position+ki-67position was an independent factors for OS in the short-term recurrence patients.. Conclusions (1) The expression of ki-67can be the indicator to evaluate the HCC recurrence after R0resection.(2) Both position of p53and ki-67in HCC tissues is an independent factors for OS in the short-term recurrence patients.
