Objective: To explore and compare the effects of total hip arthroplasty and bipolarprosthetic arthroplasty in treatment of unstable intertrochanteric fracture in the elderly.Method:The study included patients who were radomlized to take total hip arthroplastyand bipolar prosthetic arthroplasty from Third Hospital of Jilin University. Posterior-lateralapproaches and strict surgery procedures were applied. the age,the gender,the type of fracture,the operation time, the amount of blood transfusion were observed. The time of starting towalk,the hospital stays,the change of Harris Score,and postoperative complications were alsofollowed,and the functional recovery and complications is also observed.Result:The clinical data of36patients who were diagnosised as unstableintertrochanteric fracture in the elderly and needed to have artificial joint arthroplasty,totalhip arthroplasty19patients,bipolar prosthetic arthroplasty17patients, female22patients,male14patients, ranging in age from78to92,the average age is85±8.5;The Evans-Jensentype is unstable type,type Ⅲ6patients,type Ⅳ10patients,type20patients.31patients(86%)were followed3-12months. The average hospital stays of patients of total hip arthroplastygroup and bipolar prosthetic arthroplasty group were18.9±1.5days,18.4±1.1days. p=0.291>0.05, there was no obvious difference. The operation time of each group were61.1±7.1minand45.8±4.4min, bipolar prosthetic arthroplasty group was less than total hip arthroplasty.The amount of blood transfusion were2.9and3.6,p=0.03. Postoperative complications oftotal hip arthroplasty group and bipolar prosthetic arthroplasty group were2and1, p=0.36>0.05, there was no obvious difference. The bedridden time were3.9±0.8weeks and4.5±1.0weeks, p=0.097>0.05, there was no obvious difference.The decrease of Harris score is25.5±7.1,24.2±1.6,p=0.585>0.05, there was no obvious difference.Conclusion: Artificial joint arthroplasty is an practical and effective choice in thetreatment of treatment of unstable intertrochanteric fracture in the elderly.
