| BackgroundSepsis was caused by infection of systemic inflammation, which is one of the serious complications to critical patients, and is also the main causes of death in ICU. According to statistics, the number of septic patients in the United States reaches to 750,000 every year; around 200,000 patients are expected to die, and the disease rate ascend with 1.5%-8% per year. Although sepsis was a high incidence disease and with high mortality rate, the pathogenesis of sepsis is still unclear, treatments means is limited and with definitive efficacy. Former research to sepsis consider inflammation caused by excessive of organ damage, failure is the leading cause of death, with the development of research, more and more evidence show that immune disorders in sepsis plays an important role.Fish oil is life common kind of fat emulsions, which contains a large number of the omega 3 polyunsaturated fatty acids. A number of studies on animal experiment and clinical nutrition research shows that the omega 3 polyunsaturated fatty acids can change the activity of white blood cells, release cell factors and adjust the immune response, which could through adjusting immune function to treat patients with severe acute pancreatitis, acute lung injury, early sepsis& trauma and tumor.So rich in the omega more than three of the unsaturated fatty acid can fish oil to septic patients inflammation, immune function rise to adjust function, to sepsis prognosis of any apparent effect? In view of the above problem, the thinking makes the research design solutions. ObjectiveThe influence of omeiga (omega) 3 polyunsaturated fatty acid on sepsis's immune function.Research objects and methodsSelect 54 cases of septic patients who live in ICU ward of QILU Hospital of Shandong University from January 2010-April 2011 as the object of study, with the randomized, double-blind controlled and control methods, the object were divided into fish oil group and control group, each group of 27 patients. Use of nitrogen, quantity of heat, the daily calories quantity TPN 20 kcal/kg, daily nitrogen in 0.2 g/kg, total 5 d. Control group use long chain fat emulsions, fish oil groups use the add omeiga (omega) 3 polyunsaturated fatty acids in the long chain fat emulsions. In TPN support respectively before and the sixth day TPN support blood compared between the two groups of patients-6, IL CRP, HLA-DR, T cells and group of level, and APACHE II score, etc., all data are SPSS 18.0 statistical software with statistical treatment.The resultsFish oil group and control group comparison in chemical, biological, clinical features:two groups of patients age, gender, BMI and original disease has no significant difference (P>0.05), comparable.Before treatment, no significant difference (P>0.05) between two groups of patients on TPN serum IL-6, CRP level, plasma HLA-DR expression rate and T cells and group of level, blood level of WBC, APACHE II score and the SOFA score data. After TPN treatment day plus five, two groups of patients'serum IL-6, CRP level, APACHE II score and a SOFA score decreased than they are before treatment; Plasma HLA-DR expression rate and T cells and group of level are rising than before, and TPN before treatment, a statistically significant difference compared (P<0.05); Fish oil groups of patients blood levels are down before treatment WBC, in the patients blood WBC level is before treatment, and rise before treatment, the difference is TPN was statistically significant (P< 0.05). TPN after treatment compared between the two groups, the fish oil group a control group serum IL-6, CRP levels drop, blood, APACHEâ…¡WBC levels drop score and SOFA down all the more remarkable, score difference was statistically significant (P<0.05); Plasma HLA-DR expression rate and T cells and group of level no significant difference changes (P>0.05).Two groups of patients live ICU and hospitalization days total, fish oil group significantly lower than in control group (P<0.05). Two groups of patients in the hospital mortality non-significance difference (P>0.05).ConclusionAdd the omega more than three of the unsaturated fatty acid TPN can reduce sepsis patient inflammation levels, can obviously reduce ICU treatment time and the time in hospital but for the influence of immunosuppression is not clear, no significant change to mortality. |
