The Law Of Yishenhuoxue Clinical Research, Prevention And Treatment Of Kidney Stones After Extracorporeal Shock Wave Lithotripsy Kidney Injury

Background:Urinary calculus is one of the most common ailments in urinary surgery, kidney stone takes up a large proportion of them; ESWL is the main approach to treat the stone smaller than 2cm. With the extensive application of the medically shock, renal damage caused by shock wave is gradually being recognized。At present the damage is deemed to the direct action of SW, increasing of the oxygen free radical, disorder of the blood dynamics。How to prevent and treat the damage caused by ESWL, and clear the relevant factors, has became one of the hot spots in the urological studies。Calcium blockers was commonly used to reduce the renal damage, but because of its side effects, this kind of medicine in the clinical use has greatly restricted, so more attentions have paid to the Chinese medicine。Purpose:1 Observe the benefit of yi shen huo xue method to improve the symptoms and bleeding, evaluate the effect the ESWL。2 Observe the benefit of NAG andβ2-MG after ESWL, evaluate the clinical effect of yishen huoxue method; analysis the the mechanism of the herb。3 Observe the safety of the yi shen huo xue soup。Method:The test is basic on random and contrast principles。There are all 90 cases, (50 cases in treatment group,40 cases in matched group).All patients keep the urine specimen for urine tests and urine NAG andβ2-MG examination within four hours after ESWL.Treatment group take levofloxacin mesylate 0.2g bid for 3days and yishenhuoxue soup for 14 days; matched group take levofloxacin mesylate 0.2g bid for 3days.After 2 weeks, all patients observe the kidney stone by B-mode ultrasonography, reexamine the urine tests and urine NAG and B2MG.Result:By now 50 cases have already be absorbed in this test, no serious complication happens to these cases, the probability of hematuria and renal colic is nearly the same.In these 50 cases,6 cases off:2cases escape in matched group,1 case in treatment group; Grug allergy happens to 1 case in matched group,2 cases turn to the ureteroscopic stone removal treatment because of ureter steinstrasse (one case in each group).44 cases finally finish the clinical research by now (28 cases in treatment group,16 cases in matched group), The day of ESWL, red blood cell in urine:groupⅠ15.00±7.38,groupⅡ:14.80±8.08,P=0.0.887>0.05; NAG:groupⅠ9.80±4.35IU/L, groupⅡ9.27±3.50 IU/L, P=0.786>0.05;B2MG:groupⅠ1.49±1.52 mg/L,groupⅡ1.37±1.27 mg/L, P=0.708>0.05,there are no statistics differences between these two groups. By observing the size the kidney stone, we find ESWL works 96.43% in treatment group,93.75% in matched group, but in treatment group 78.57% kidney stones are drained, in matched group this number is 56.25%, it proves that yishenhuoxue soup helps to drain the stone. After two weeks, the RBC,NAG,B2MG in urine all improve: RBC:GroupⅠ13.10±6.55, GroupⅡ12.67±7.10, P=0.350>0.05 no statistics differences happen to them; NAG:GroupⅠ3.66±3.03 IU/L, GroupⅡ1.84±1.75 IU/L, P=0.006<0.05; B2MG:GroupⅠ1.01±0.94 mg/L, GroupⅡ0.53±0.45 mg/L, P=0.016<0.05, There are significant differences between them.Conclusion:By this clinical research, firstly confirms that the infection of the shock-wave treatment is curative and safe, a total of 90% or more efficient, and no serious complications By comparing the rate of stone drained we find that yishenhuoxue soup helps to drain the stone. Two weeks after ESWL the counts of RBC,NAG,B2MG in urine have a remarkably improved in each group; it means the kidney has the ability to repair itself. But the treatment group help to improve NAG and B2MG more obviously(P<0.05), we confirm that yishenhuoxue soup exactly work to cure the renal damage caused by ESWL...
