Objective:The purpose of this study was to investigate the correlativity expression of VEGF(vascular endothelial growth factor) and PEDF(pigment epithelium derived factor) in NSCLC(non-small cell lung cancer)tissue, their relations with MVD(micro vessel density) in cancer tissues and with the biological behavior and prognosis of NSCLC.Methods:1. The specimens were from 50 NSCLC patients who were diagnosed pathologically and operated in our hospital from 2007 to 2008.2. The standard of specimens: There was no radiotherapy or chemotherapy before operation. To classify these cancer tissues by gender, ages, clinic pathological stages, types of microanatomy and lymph node metastasis. Follow-up lasted until February, 2011.3.Tests of specimens: These 50 lung specimens were analyzed by MaxVision immunohistochemistry method to investigate the expression of VEGF,PEDF and CD31 antibody.4. Immunohistochemical evaluation was performed by a pathologist in a blind test. At least 10 high-power fields from a single tissue section were chosen at random. Positive cells were stained dark brown on the cytoplasm. The intensive score was categorized into the following 3 groups: 0, no staining; 1, mild staining; 2, marked staining. The sum score of positive cell was classified into 4 groups: 0, less than 10% positive; 1, 10%~40% positive; 2, 40%~70% positive; 3, more than 70% positive. The product of intensity score and sum score was classified as following 3 grades: negative, product score was 0; mild positive, product score was 1-3; marked positive, product score was 4.5. Test of MVD: Staining the vascular endothelial cells by CD31 antibody and counting the MVD. Using the average value of 4 high-power fields as MVD of this specimen.6. All statistical analyses were conducted by SPSS for windows 19.0 statistical software system. Comparison of positive rate using the method ofΧ2 test. Spearman Correlation analysis to judge the relation between VEGF and PEDF. The influence of each variable on survival was assessed by the COX model.Results:1. In the 50 pulmonary cancer tissues, VEGF was expressed in 32 cases (64.00%), mainly located at cytoplasm or the membrane of lung cancer cells; PEDF was expressed in 22 cases (44.00%), mainly located at cytoplasm or the membrane of NSCLC cells;2. The MVD was significantly greater in VEGF positive groups than that in negative groups (P<0.05); but the MVD was lower in PEDF positive groups than that in negative groups (P<0.05); both VEGF and PEDF were well correlated with lymph node metastasis state (P<0.05) and clinical pathological stage (P<0.05); 3. 45 follow-up data among the 50 patients were completed with 25 cases whose life span were more than 3 years and the other 20 cases less than 3 years. The expression of PEDF and VEGF was negatively correlated r=-0.599 (P=0.000).4. Through the Kaplan-Meier survival analysis, we can get these results: the VEGF positive groups` survival rate lower than VEGF negative groups and PEDF positive groups` survival rate higher than VEGF negative groups, the differences have statistical significance. (χ2=17.99,P=0.000, Log-rank Test) Conclusion:The result of the study indicate that the expression of VEGF was highly in NSCLC than in begin tumor-like tissues, but the expression of PEDF was lower in NSCLC tissues, and they were positively correlated with lymph node metastasis state and clinical pathological stage (all P<0.05). In addition, the MVD was significantly greater in VEGF positive group than in negative group (P<0.01). The expression VEGF and PEDF was negative correlated r=-0.599 (P=0.000). Thus VEGF and PEDF can be used as a major parameter for detecting biological behavior and prognosis of NSCLC to judge the angiogenesis, invasion, metastasis from lymph node and prognosis of NSCLC, and VEGF, PEDF can be used to assess the prognosis of NSCLC patients, and they can be used as the new target for the treatment of NSCLC.
