ObjectiveWe carried out health propaganda and a series of intervention measures for TB patients to increase the co-infection case finding rate, we also evaluate the effectiveness of health promotion, to analyze the key factors which influence the behavior to do HIV antibody test. Then the reasonable measures would be suggested to the TB-HIV co-infection controlling and detect more HIV-infectors among TB patients.MethodsWe investigate the newly diagnosed tuberculosis patients by questionnaires covering Suizhou city,Nanzhang county, Ezhou city of Hubei province by typical sampling according to the TB incidence to understand the co-infection status and main factors which influence the behavior to do HIV antibody test, and take publicity and education by medical workers for six months to compare the HIV testing rate and co-infection knowledge.ResultsⅠ1879 TB patients joined the survey, 14 cases were diagnosed HIV positive. Co-infection rate was 0.74% percent. The characteristic of the 14 co-infection patients were: male, newly diagnosed TB, peasants, about thirty to fifty years old, high school level, of which 11 patients had paid blood or blood transfusion history.ⅡThe willingness rate of HIV testing was higher than the acceptance rate in the TB patients. No idea of the state of co-infection, thinking it necessary for HIV voluntary counseling and testing, doctor education, patients in Suizhou and Nanzhang area were more likely to receive HIV voluntary counseling and testing in single and multiply Logistic regression. Ⅲ10% of the patients knew the prevention and treatment knowledge of tuberculosis and HIV. HIV testing rate was improved obviously after health promotion(P<0.05). The knowledge of tuberculosis, AIDS, co-infection, the necessity of HIV testing and national policies in TB patients coming from Suizhou, Nanzhang area has significant ris(eP<0.05), patients'knowledge of tuberculosis were increased in Ezhou city(P<0.05), which of co-infection, national policies on TB and AIDS were dropped ( P>0.05 ) , the accteptance rate is higer than previous investigation.ConclusionⅠThe co-infection rate is relatively high in Hubei province. Blood transfusion is the main way of infecting HIV/AIDS. We should put focus on testing low level of economy and education degree, young male, newly diagnosed TB patients.ⅡNational policy and doctors'education affected the HIV testing rate of TB patients in a certain degree. We should intensify publicizing the importance of HIV testing and preventing AIDS, doing fully confidential, social and psychological support work to improve the HIV positive detection rate in TB patients.ⅢThe co-infection knowledge rate of TB patients was still low. Various health promotions aimed at different targeted patients can better improve the understanding and significance of knowledge, altitude and behavior of HIV testing and detecting in TB patients.
