PurposeTo investigate abdominal US findings of Disseminated Penicilliosis Marneffei in patients with AIDS and Ultrasound diagnostic application value.MethodsAbdominal US imaging of Disseminated Penicilliosis Marneffei in 177 patients with AIDS were contrastive retrospectively analyzed with 177 HIV- positive patients without Opportunistic infection and tumor.8 cases in the 24 cases of focal liver's lesions performed Contrast-enhanced Ultrasound. The abdominal US findings were analyzed after antifungal therapy 1-2 months , then summaried the changes of abdominal lesions.ResultsEnlargement of abdominal lymph nodes were observed in 79(44.63 % ) cases.Hepatomegaly was observed in 57(32.20%) cases.Splenomegaly was observed in 88 (49.71%) cases .Focal or diffuse lesions in the liver and spleen were observed in 48 (27.12%)cases .4 case of diffuse lesions in the liver or spleen whose image was very specific "cloudy". Gallbladder wall thickening was observed in 19 (10.73%) cases. Cholecyslithiasis was observed in 15(8.47%) cases.Pancreatitis was observed in 6(3.39%)cases. Ascites was observed in 36 case(20.34%).The above findings except the emergence of fatty liver have statistically significant difference (P < 0.05).7 cases of arterial were isoechoic, portal and delay period were slightly isoechoic in 8 cases of CEUS. 1 case centre parts has not seen the enhancement in three phase , peripheral are isoechoic in three phase. Area without enhance corresponding completely necrosis,enhanced area corresponding liver tissue and granulation tissue.177 cases of AIDS merger PSM patients had 5 cases's death due to lung infection serious causing respiratory failure over short-term . 172 cases examined after treatment. Abdominal swollen lymph node disappeared or quantity and size obviously reduced. Hepatomegaly ,Splenomegaly and focal lesions in the liver and spleen had different rate reduced. Ascites disappear or reduced. Gallbladder wall was thinning. Pancreas lessened.ConclusionsDisseminated Penicilliosis Mameffei in patients with AIDS usually were involved multiple organs in abdomem. Enlargement of abdominal lymph nodes,hepatosplenomegaly ,parenehymal lesions in the liver and spleen are the most common US findings in abdomem. CEUS's finding of focal lesion in the liver and pathological changes have close relationship ,and has certain differential diagnosis's value with other liver of focal lesions.Ultrasound examination is readily available and convenient,which can provide more information for the diagnosis and observation of the patients with Disseminated Penicilliosis Marneffei.
