Objective:Cervical cancer was first in gynecological Malignant tumors and early surgery was the main method of treatment for cervical cancer. There were some psychiatric symptoms such as anxiety and depression for perioperative gynecologic malignancies. The occurrence of malignant tumors and their survival time of patients were for psychological factors. Psychological intervention help mechanisms of the nervous system to restore and patients were stirred the corresponding changes in the neuroendocrine system so as to achieve the purpose of treatment by starting its own immune system. In the study, it was taken cognitive and behavioral inspire patients with Chinese characteristics into the model of psychological intervention by nurses so that the patients with negative emotion were elimination and it achieved the lifting of anxiety, depression, psychological distress and improved immune function. Thus, it was provided a more in-depth psychological intervention, more effective method for the patients with cervical cancer.Method: The patients diagnosed with cervical cancer were randomly divided into psychological intervention group and conventional nursing group. The conventional nursing group received only conventional mental health care and education in general diseases, but the intervention group were additional received psychological intervention. The two groups were interviewed the first day before surgery and they filled out the questionnaire, self-rating anxiety scale (SAS) and self-rating depression scale (SDS), then they were also collected peripheral venous blood coagulation test against TNF-a, T cells subgroup level. After 14 days, they should complete self-rating anxiety scale (SAS), self-rating depression scale (SDS) psychological test assessment, and collection of peripheral venous anticoagulant test T cell subsets, TNF-a levels. Compared the effects between psychological intervention group and conventional nursing group.Results:1. The two groups in age, marital status, education level, occupation, economic conditions had generally no significant differences in the clinical data and the two groups were comparable.2. We took the multivariate stepwise regression analysis with the total score of emotion as the dependent variable while age,marital status,and education level as the independent variable. The result indicated that age,marital status,and low education level had influence on anxiety and depression scores of patients. Patients which are young,divorced/separate or at low education level have high scores of anxiety and depression. 3.Compared the scores mean of anxiety and depression between the two groups before and after intervention and there were significant differences (P<0.05). And the anxiety and depression of the experimental group after the intervention of psychological were improved significantly.4.Compared the anxiety and depression of the experimental group before and after psychological intervention: The patients after intervention with anxiety and depression scores were lower than that before intervention, and the statistical analysis showed significant differences. (P <0.05)5. The comparison of T lymphocyte subsets between two groups of patients before and after intervention: compared the values of CD3~+,CD4~+,CD8~+,CD4~+/CD8~+ between the two groups before intervention,P values were all more than 0.05 with statistical analysis,so there was no statistically significant differences. But the patients of experimental group after intervention with T-cell subsets in the index content increased significantly,and the average were all higher than contrast group,but CD8~+ was lower than the contrast group, then the statistical comparison CD3~+,CD4~+,CD8~+,CD4~+/CD8~+, P values were Less than 0.05,so it had statistically significance. The comparison proved that the immune status had been improved after psychological intervention.6.Compared T lymphocyte subsets of the experimental groups between before and after psychological intervention: the P values of CD3~+,CD4~+,CD8~+,CD4~+/CD8~+ were all less than 0.05 with statistical analysis,which possessed statistically significant differences. It indicates that the psychological intervention have certain effect on those patients' immune improvements.7.Compared TNF-αconcentration (ng / L) between the two groups of patients before and after intervention: The patients'TNF-αconcentration between two groups before psychological intervention were no statistically differences(P>0.05). Compared the TNF-αconcentration (ng / L) of experimental and contrast groups after psychological intervention, it had statistically significant differences(P <0.001). The TNF-αconcentration of the experimental group patients were significantly reduced compared with contrast groups after intervention. It indicates that surgery as a kind of method can reduce the patients' immune function, and psychological intervention can improve patients' immune function.Conclusion: The patients with cervical cancer have anxiety,depression and other negative emotions. Patients which are young,divorced/separate or at low education level have higher anxiety,depression scores. The negative emotions are correlated with immune function. The patients with cervical cancer after psychological intervention could face the reality positively,self-adjust,rebuild social adaptability and actively participate in treatment,improve survival and quality of life,return to society, join in the family. Thus,it provides a more in-depth psychological intervention,more effective method for the patients with cervical cancer. |
