With the rapid economic development, social improvement and lifestyles changement, the incidence of Metabolic Diseases such as obesity and diabetes (DM), increases rapidly in the worldwide. Fllowing heart Cerebrovascular disease and Cancer, the Metabolic Diseases has become the third chronic non-communicable diseases which threaten human health severely. Numerous studies conducted these few years have revealed that obesity can be extended from childhood to adults, which can contribute to high incidence of cardiovascular disease and diabetes. To some degree, Childhood are at the initial stage of some adult chronic disease. Nonetheless, obesity can causes insulin resistance (insulin resistance, IR) easily in obese patients, while the major mechanisms are adipose tissue endocrine disorders and ectopic fat deposition. Recently studies also suggest that adipose tissues serve as much more important endocrine organs besides its's vital role in metabolism and storage of energy. Compared to subcutaneous adipose tissue, the visceral adipose tissue display much more powerful ability of adipocytokines secretion. And vaspin can enhance insulin sensitivity, was discovered from visceral adipose by Hida in 2005. Here we study the relationship between vaspin and insulin sensitivity, glucose and lipid metabolism in obese children and young mice, elucidate the effect of vaspin on insulin sensitivity, glucose and lipid metabolism.Chapter 1 Association between vaspin concentrations and insulin sensitivity, glucose and lipid metabolism in the visceral adipose tissue of obese miceObjective:To investigate the relationship between vaspin concentrations and insulin sensitivity, glucose and lipid metabolism in the visceral adipose tissue of obese mice.Methods:24 weaned rats were randomly divided inio two groups,one group is with 12 rats (6 males and 6 males) received normal diet as control while the experimental group (6 males and 6 males) is given high fat diet for 4 weeks.The weight,length, abdominal circumference(AC), systolic blood pressure(SBP), diastolic blood pressure(DBP) were measured using standard instruments and procedures, while the blood biochemical index including fasting plasma glucose(FPG), fasting insulin(FINS), triglyceride(TG), total cholesterol(TC), high density lipoprotein-cholesterol(HDL-C), low density lipoprotein-cholesterol(LDL-C) were detected using the collected venous blood of the inner canthus.Then blood glucose and insulin were tested again at different time point after the glucose (at a dose of 2g/kg) was injected into the abdominal of the rats.The liver and the fat tissue which come from the perinephric fat were measured after rats were killed by cervical dislocation.The vaspin concentrations of the visceral adipose tissue was detected by ELISA,and compareed with the insulin sensitivity, glucose and lipid metabolism associate factors.Results:The FC group rats have much more fatter shape. And the weight, AC, the weight of visceral adipose tissue and liver, SBP, FPG, FINS,120minINS, vapsin, TC, TG, LDL-C, LDL-C/HDL-C, (TC-HDL-C)/HDL-C, HOMA-IR of the FC group were much higher, while the HDL-C, ISI were much lower than the NC group. There is a significant positive correlation between the vaspin concentrations and the weight of visceral adipose tissue and liver, AC, TG,120minINS, LDL-C/HDL-C, (TC-HDL-C)/HDL-C, TC, LDL-C, FPG, FINS, HOMA-IR, While a negatively correlated with HDL-C, ISI.Conclusion:The obesity rats model was successfully established, higher vaspin concentrations of visceral adipose tissue, insulin resistance, glucose and lipid metabolism disorders were found in FC group, and vaspin concentration was strongly associated with obesity and insulin resistance.Chapter 2 Association between serum vaspin concentrations and insulin sensitivity, glucose and lipid metabolism in the obese childrenObjective:To investigate the relationship between serum vaspin concentrations and insulin sensitivity, glucose and lipid metabolism in the obese children. Methods:A total of 34 obese children (22 males and 12 females) were enrolled in this trial. And another randomly selected 30 cases of normal healthy children were taken as conctrol. Fasting venous blood samples were collected for the determination of the serum vaspin level by Enzyme-linked immunosorbent assay (ELISA). The Physical index such as anthropometric weight, height and the FPG, FINS were measured using standard instruments and procedures, and the body mass index (BMI), Waist-hips-ratio(WHR), body fat%(BF%), HOMA-IR, ISI and AI were caculated according to suitable formula, the relationship between serum vaspin concentrations and insulin sensitivity, glucose and lipid metabolism was investigated.Results:The vaspin, WC, BMI, BF%, SBP, DBP, FPG, FINS, LDL-C, TG, LDL-C/HDL-C, (TC-HDL-C)/HDL-C, HOMA-IR of the obese children were much higher, while the HDL-C, ISI were much lower than the control group. There is a significant positive correlation between the vaspin concentrations and BMI, WC, WHR, BF%, TG, FINS, HOMA-IR, (TC-HDL-C)/HDL-C, DBP, While a negatively correlated with HDL-C, ISI. Multiple linear regression suggests that BF% is an independent risk factor for the serum vaspin concentrations.Conclusion:Metabolism disorders such as high blood pressure, glucose and lipid metabolism disorders, insulin resistance were found in the obese children. And the serum vaspin concentration was strongly associated with obesity and atherogenic index. Monitoring the serum vaspin levels may have a very important value for understanding the degree of obesity and insulin resistance.
