| Objective:In order to sum up the experience on diagnosis and treatment of primary gastrointestinal lymphoma(PGIL) by retrospective analysis of single centre 22 cases.Method:22 patients admitted to the Department of General Surgery, Sir Run Run Shaw Hospital, during 1997 to 2009 were analyzed retrospectively. Their clinical manifestations, diagnosis, treatment and prognosis were analyzed. Results:There were 22 cases of gastrointestinal lymphoma as a result,14 cases in stomach,4 cases in small intestine,4 cases in colorectal and rectum. Only 6 cases (27%) were diagnosed by endoscopic biopsy preoperative, while the other 16 cases where diagnosed after surgery. There were 13 cases in stage IE &â…¡E,9 cases in stageâ…¢E &â…£E.6 patients were treated by surgery combined with chemotherapy,2 patients were treated by surgery only. 3 patients were treated by palliative surgery combined with chemotherapy,11 patients were treated by chemotherapy only. Until now 6 cases living with tumor,6 cases living without tumor. In stageâ… E &â…¡E, the 5 years survival rate of the surgery group is 75% (6/8), with 2 cases living with no tumor. Average survival time is 97 months. The 5 years survival rate of the chemotherapy group is 40% (2/5), with 1 cases living with no tumor. Average survival time is 97 months. In stageâ… E &â…¡E, the 2 years survival rate was 44.4%(4/9), the average survival time is 22 months. Conclusion:The incidence of primary gastrointestinal lymphoma is low, and it mainly is on stomach. The main pathological type is diffuse large B cell lymphoma. Clinical symptoms are nonspecific, just abdominal pain. Endoscopic biopsy is the main method for preoperative diagnosis, but very difficult. The treatment of early stage PGIL is surgery combined with chemotherapy, while for advanced stage PGIL is comprehensive treatment of chemotherapy combined with radiotherapy. The prognosis is better than the gastrointestinal cancer. |
