Objective To observe the change of serum transaminase level after hepatectomy and discuss the influencing factors.Methods To record the factors on the consecutive 66 patients underwent hepatectomy, such as surgical method,time of hepatic pedicle clamping,degree of hepatic transposition, acreage of hepatic section,cirrhosis and the serum transaminase valve on the first day,the third day and the seventh day after hepatectomy.Multivariate stepwise regression and covariation model were used to analysis the relation between the factors and serum transaminase level after hepatectomy.Result The rising of serum transaminase was brief after hepatectomy,the peak valve was on the first day after hepatectomy,than dropped gradually.By Pearson correlation analysis,the relationship was found between the rising of serum transaminase after hepatectomy and the hepatic pedicle clamping,hepatic transposition,cirrhosis,acreage of hepatic section and surgical method in influencing(p<0.05).Four of them,the hepatic pedicle clamping,hepatic transposition,cirrhosis and surgical method,entered the regression equation.Conclusion Both hepatic inflow occlusion and hepatic transposition were major factors causing the rising of postoperative serum transaminase level.The time of hepatic inflow occlusion and the severity of cirrhosis were major factor causing the persistent rising of postoperative serum transaminase level.There was a relative between the surgical methods and serum ALT level on the first day and third day after hepatectomy momently.
