Objective:The purpose of this study to further explore the expression of cardiac NHE1 mRNA and myocardial fibrosis in the effort failure probably happened in the development of the role and significance by measuring myocardial tissue in patients with heart failure and NHE1mRNA expression in peripheral blood of patients with target-related myocardial fibrosis(PⅠCP,PⅢNP and ET).Methods:35 patients with heart failure were enrolled in our study,The basis of NYHA classification standards,the above-mentioned cases will be divided into cardiac function classⅠ~Ⅲthree groups,one of YHAⅠgroup of 11 cases, NYHAⅡgroup of 12 cases,NYHAⅢgroup of 12 cases.The right atrial tissue samples were taken from 25 patients with cardiac surgery.The mRNA amounts of NHE1 were studied by Real-time Fluorescence Quantitative PCR(FQ-PCR) method. The concentrations of plasma levels of endothelin(ET),serum levels of carboxyterminal propeptide of typeⅠprocollagen(PⅠCP) and aminoterminal propeptide of typeⅢprocollagen(PⅢNP) were measured by radioimmunoassay (RIA) in35 patients of heart failure and 25 controls.Results:1.Compared with NYHAⅠgroup,the mRNA amounts of NHE1 in NYHAⅡgroup and NYHAⅢgroup were significantly higher(P<0.01).at the same time, Compared with NYHAⅡgroup,the NHE1 mRNA level in NYHAⅢgroup was higher,and therewas statistically significant difference(P<0.05).the level of NHE1 mRNA was up-regulated significantly with cardiac function class classified by New York Heart Association.2.The levels of endothelin(ET),carboxyterminal propeptide of typeⅠprocollagen(PⅠCP) and aminoterminal propeptide of typeⅢprocollagen(PⅢNP) in group of HF were significantly higher than the control group(P<0.01).at the same time,the levels of ET,PⅠCP and PⅢNP were up-regulated significantly with cardiac function class classified by New York Heart Association.By one-way ANOVA,ET, PⅠCP,PⅢNP expression in the all group have a difference,and there is statistical significance(F=8.99,P<0.01;F=8.372,P<0.01;F=10.11,P<0.01).Compared with NYHAⅠgroup,the levels of ET,PⅠCP and PⅢNP were higher(P<0.05;P<0.01), Compared with NYHAⅡgroup,the all levels were increased(P<0.01).3.by Pearson correlation analysis,ET and PⅠCP,PⅢNP was a positive correlation(r=0.574,P<0.001;r=0.596,P<0.001).PⅠCP and PⅢNP content was also positively correlated(r=0.688,P<0.001).Conclusion:1.The change of expression of NHE1mRNA in patients with HF was possibly one of the molecule mechanisms of myocardial remodeling and related with the genesis and maintenance of heart failure.2.The levels of myocardial fibrosis markers increase in patients with CHF. These results indicate that myocardial fibrosis may take place in heart failure of the patients.
