Integrative Medicine Systematic Reviews Of Conservative Medical Treatment For Ectopic Pregnancy

[object] :Through evaluating the present condition and residual problems in the scientific research design and therapeutic effects of the clinical studies on conservative combined treatment of traditional Chinese medicine and western medicine to ectopic pregnancy, this paper provides a evaluation on the level of research qualities in treating ectopic pregnancy. It employs the systematical evaluation methods to make a subjective appraising to the effectiveness and safety of conservative combined treatment of traditional Chinese medicine and western medicine to ectopic pregnancy.[methord]:After collecting randomized comparison clinical research literature review , this paper sets up an evaluation table of the clinical research articles about treating ectopic pregnancy conservatively with Chinese and western medicine. It evaluates the quality of relating articles, according to Jadad quality appraising methods. The statistical analysis is based on four aspects: general condition of literatures, criteria of diagnosis, inclusion and exclusion, criteria of effect judgment and research design appraising. With the statistical analysis thereon that it does Meta analysis with software of review manager (manager 4. 2).[Results]: For the 108 articles referring to randomized controlled trial, 93 articles (86. 3%) have clear diagnosis criteria, 43 articles (39. 8%) have acute inclusion criteria, and 98 pieces (91. 8%) are clear in the criteria effect judgment. Nevertheless, the quality of RCT is generally low, only one document mentioning the random method, only one document employing single-blind, and none referred to the hidden distribution. For the description of Group balance , 70 articles (64.8%) have clear words about baseline comparison for general conditions, 93 pieces (86.1%)mention the right statistical method, 48 pieces (44. 4%) report adverse effect, and 41(37.9%) pieces have follow-up visit. Meanwhile, there are 39 articles adopted in the systematic review, in which 16 exploit methotrexate combined with traditional Chinese medicine treatment compared methotrexate treatment alone, 2 make contrast between methotrexate combined with traditional Chinese medicine treatment and traditional Chinese medicine treatment, 6 compare the effects of adopting mifepristone treatment alone and mifepristone treatment combined with traditional Chinese medicine, and there are 9 articles comparing the treatment effects of Mifepristone combined with methotrexate and traditional Chinese medicine with the treatment effects of Mifepristone combined with methotrexate. In contrast , Clinical efficiency, the time required for B-HCG blood becoming normallity, the time required for Annex mass to disappear have significant difference (P<0. 01). The asymmetry showed in "Inverted funnel plot" may be related to the publication bias and the design of clinical experiment.[Conclusion] the systematic review shows that conservative combined treatment of traditional Chinese medicine and western medicine to ectopic pregnancy is more effective than lonely using. However, as insufficiency and low quality of the samples adopted, as well as the potential publication bias, this system cannot yet draw an affirmative conclusion to the efficacy and safety of traditional Chinese medicine and western medicine in the treatment of ectopic pregnancy, which should be further evaluated by more rigorously designed randomized, multiply-center trails, so as to promote the modernization of Chinese medicine.
