Chronic Fatigue Syndrome Tcm Diagnosis

Objectives:According to the basic theory and method of Chronomedicine diagnose of TCM research the characteristic of the CFS's location and nature to provide experimental evidence for CFS clinical differentiation.Methods:Collected 20 cases CFS patients and at same period collected 20 healthy controls.Used dynamic test method of chronobiology to consecutive 24 hours dynamic testing the subjects body temperature,heart rate,blood pressure,salivary cortislo and melatonin.Application Cosinor Method Analysis to analysis the micro rhythm characteristic,used Repeated Measures,Independent -Samples T Test to analysis the macro features,thus acquiring the daytime-night variation characteristic of the all detection indications,analysis those characteristics which are contained the features of TCM' syndrome.According to the basic theory and method of Chronomedicine diagnose of TCM research the characteristic of the CFS's location and nature.Results:①The CFS patient's temperature has a characteristic of heat in sunset and cool in morning.According to the "daytime-night Yin and Wei running rhythm" theory deduce CFS have Yin deficiency and blood stasis.②The 24-hour general variation tendency of CFS patient's heart rate,blood pressure,cortisol compared with the healthy group there is no distinctly difference(p＞0.05).However,two marker rhythm: temperature and melatonin show the features of midnight to morning aggravation. According to the theory of "daytime-night increase-descend of Yin and Yang rhythm" deduce CFS has Qi deficiency.③According to the "daytime-night Five Zang matching time rhythm" theory,the temperature of CFS patients show abnormal at 4:00(p＜0.05),it occurred in the "Pingdan" means spleen deficiency;heart rate show abnormal at 5:00 and 8:00(p＜0.05),it occurred in "Pingdan" and "Siji" means spleen and kidney deficiency;systolic pressure show abnormal at 8:00 and 17:00(p＜0.05),it occurred in the "Siji" and "Xiapu" means liver and kidney deficiency;disstolic pressure show abnonual at 20:00(p＜0.05),it occurred in the "Siji" means kidney deficiency;salivary melatonin show abnormal at 0:00(p＜0.01),it occurred in the "Yeban" means heart deficiency.The 24-hour general variation tendency of temperature and melatonin show abnormal at same time inferred heart and spleen deficiency plays an important role in the CFS formation.Conclusions:①CFS has the characteristic of Qi deficiency,Yin deficiency and blood stasis stagnation.②The position of CFS related to heart,spleen,liver and kidney, but the clinical differential should focus on the heart and spleen deficiency.
