Through A Single Nostril - Transsphenoidal 245 Patients With Pituitary Adenoma Analysis

Objective: To analysis the superiority of direct transnasal transsphenoidal approach for 245 pituitary adenomas. The surgical multimodality treatment can help diagnosis, raising the cure rate, reducing the rate of complications and the recurrant rate,improving the social psychological adaptation of patients.Method:245 inpatients were chosen to review from April 2005 to April 2007 in the Department of Neurosurgery of the Affiliated Hospital of Qingdao University Medical Colledge, that had been diagnosed as pituitary adenomas by pathology,and whose clinical dating were complete., Each patient has received MRI or CT and pituitary function examination. All of them have received the direct transnasal transsphenoidal approach surgery.40 pituitary adenomas received transpterional operation. Two groups are compared on surgery time,effect after opration,the complications and so on.Result: 1. There is no case of death among 245 patients. The rate of total excision is about 85.31%, 115 cases in 123 patients got their vision and view improving,the rate of the improvement is about 93.50%. 75 cases in 78 patients got their level of the hormone reducing , the rate of the improvement is about 96.15%,87 cases got diabetes insipidus,occupy 35.51%,the longest period in hospital cost 23 days,the shortest is 5 days,the average period is 11 days . 2. In the transpterional surgery group members, the rate of total excision is about 77.5%, 18 of the transpterional surgery group members had the vision and view problem, 16 of them improved after operation, the rate of the improvement is about 88.89%. 23 cases in 28 patients got their level of the hormone reducing , the rate of the improvement is about 82.14%, 18 cases got diabetes insipidus, occupy 45%, the average period in hospital cost 17 days.Conclusion: The pituitary adenoma resection by trans-single nostriltranssphenoidal sinus approach,which is used to completely rescet with lesstrauma and fewer complications,is a reliable and effective method, easy to perform.
