Type 2 Diabetes In Patients With Carotid Artery Intima-media Thickness And Insulin Resistance

Objective To evaluate the relationship of carotid endomembrane thickening with insulin resistance and comphyperinsulinemia in type 2 diabetic subjects.Methods Ultrasound quantification of carotid IMT was performed in 105 patients of type 2 diabetes.These patients were further subdivided into three groups:group A(IMT＜0.9mm,34 subjects),group B(0.9mm≤IMT＜1.3mm,49 subjects),group C (IMT≥1.3mm,35 subjects).42 patients without diabetic mellitus were enrolled as control group.Their blood pressure and body height and weight were measured(BMI=body weight²/body height).Their plasma lipids,hs-CRP,FINS,FPG,LDL,HDL and insulin were also measured.Resistance index HOMA-IR=(FPG×FINS)/22.5.The data was compared and analyzed.Results The BMI,FINS and lnHOMA-IR,TC,HDL-C and IMT in group T₂DM were significant increased compared to that in control group(t=2.266,2.243,5.208,6.556,6.254,2.629,3.482,p＜0.05)..In group B and group C,the course of disease,SBP,FINS,2hPG and lnHOMA-IR were significant higher than in group A. In group C,LDL-C were significant higher than in group A and group B(p＜0.05).Single regression analysis confirmed that carotid IMT was correlated with age and LDL-C in control group.In type 2 diabetic subjects carotid IMT was correlated with course of disease,FINS,2hPG,lnHOMA-IR (r=0.266,0.316,0.207,0.301,p＜0.05)..Multiple regression analysis confirmed that carotid IMT was correlated with the course of disease and FINS and lnHOMA in type 2 diabetic patients(p＜0.05). Conclusion Insulin resistance and comphyperinsulinemia are important factors for carotid arteriosclerosis in type 2 diabetic subjects.
