| Part One: Background On AsthmaBronchial asthma is a major worldwide health problem that has an increasingprevalence and mortality. Chronic airway inflammation and remodeling arecharacteristic features of patients with bronchial asthma. Airway remodeling leads toirreversible bronchial obstruction, which may make the treatment of asthma difficult.Airway wall thickening consists mainly of hypertrophy and hyperplasia of smoothmuscle, an increase in mucous gland, infiltration of inflammatory cells, extracellularmatrix tissue edema, and angiogenesis in the bronchial wall. Hormone is one of auseful medicine to control asthma; however some patients are difficult to treat.Traditional Chinese medicine has a long history on asthma treatment, which hasabundance experience. We have use traditional Chinese medicine treating somepatients with bronchial asthma, which come to good results.Part Two: Clinical Study On The Effect Of MaHong Decoction On AsthmaPersistentObjective: The effect of MaHong Decoction on bronchial asthma patients.Method: In a randomized, single-blind, controlled study, 64 patients are randomlyselected to receive either of the following treatment for 14 days: MaHong Decoctionor the western medicine (hormonal et al). Then we analyzed the total curativeeffect ,the the symptoms and the force expiratory volume in one second (FEV1) , thepeak expiratory flow (PEF) and FEV1/FVC were measured as the primary effectivevalues before and after the therapy.Result: After two weeks treatment, lung function improvement: In both groupssignificant improvements in FEV1 and PEF were found (P<0.01) , FEV1/FVC hadimproved significant in the western medicine group (P<0.05) while insignificant inMaHong Decoction group (P>0.05) ; The symptoms were improved in both groups(P<0.01) ; The difference between two groups was significantly in symptoms (P<0.05) ,MaHong Decoction group was better than the western medicine group. The totlecurative effect were similar between two groups (P>0.05) .Conclusion: MaHong decoction can improve the lung function of bronchial asthmapatients mean while MaHong decoction can reduce the symptoms of clinic which wasbetter than the western medicine. Part Three: Experimental Study On The Effect Of MaHong Decoction OnAsthma PersistentObjective: The influence of MaHong Decoction on rat's bronchial asthma airwayremolding model.Method: Eighty male SD rats (200±10g) were randomly divided into eight groups:blank group, nature group, asthma group, hormone group, Chinese medicine group(big), Chinese medicine group (small), hormone and Chinese medicine group (big),hormone and Chinese medicine group (small), 10 rats in each group. Except blankgroup and nature group, other groups was sensitized and challenged with ovalbuminto establish the rats' asthma model and the rats were conferred medicine before thechallenge. ELISA was used to measure the level of rat's serum VEGF and b-FGF,while RIA was used to measure the level of rat's serum ET. The lung tissue of VEGF,b-FGF and MVD expression were detected with immunohistochemical ABC method.Result: (1) The serum VEGF and b-FGF in asthma group was significantly higherthan other groups (P<0.01) ; The level of serum VEGF in blank groups were greatlylower than medicine groups (P<0.05) ; The level of serum VEGF in hormone groupwere significantly increased compared to those in blank groups (P<0.05) ; The level ofserum VEGF in medicine groups were similar (P>0.05) . The level of serum b-FGF inmedicine groups were similar (P>0.05) . (2) The serum ET in asthma group wassignificantly higher than hormone and Chinese medicine groups (big), blank groupsand Chinese medicine groups (big) (P<0.01) ; The level of serum ET in asthma groupwere increased compared to those in hormone and Chinese medicine group (small),Chinese medicine group (small) and hormone group (P<0.05) ; The level of serum ETin medicine groups and blank groups were similar (P>0.05) . (3) The lung tissue ofVEGF positive cells in asthma group were significantly increased than other groups(P<0.01 or P<0.05) ; And the blank groups were lower than other groups (P<0.01 orP<0.05) ; The number of VEGF positive cells in lung tissue were similar betweenmedicine groups (P>0.05) . (4) The lung tissue of b-FGF positive cells in asthma groupwere significantly increased than other groups (P<0.01) ; Chinese medicine group(small) were lower than asthma group and higher than other groups (P<0.01) . (5) Thelung tissue MVD expression of asthma group were significantly increased than othergroups (P<0.01) ; the blank groups were romantically lower than other groups(P<0.01) ; the expression between medicine groups were similar (P>0.05) . (6) Inverse correlations were found between the serum VEGF level and the serum b-FGF level(r=0.505, P=0.000) ; Inverse correlations were found between the serum VEGF leveland the lung tissue of VEGF positive cells( r=0.461, P=0.000) ; Inverse correlationswere found between the serum b-FGF level and the lung tissue of b-FGF positivecells( r=0.453, P=0.001) ; Inverse correlations were found between the serum VEGFlevel and the lung tissue of b-FGF positive cells (r=0.644, P=0.000) ; No correlationswere demonstrated between the serum b-FGF level and the lung tissue of VEFGpositive cells; (7) Also the serum of VEGF and b-FGF level were significantlycorrelated with the lung tissue MVD (r=0.666, P=0.000; r= 0.439, P=0.001) ; (8) Inverse correlations were found between the serum of ET level and the the serum ofVEGF and b-FGF level (r=0.332, P=0.009; r=0.340, P=0.007) ; Inverse correlationswere found between the serum ET level and the lung tissue of VEGF positive cells(r=0.275, P=0.040) ; No correlations were demonstrated between the serum ET leveland the lung tissue of b-FGF positive cells; (9) Also the serum of ET level weresignificantly correlated with the lung tissue MVD (r=0.398, P=0.003) .Conclusion: MaHong decoction may control the level of rat's serum VEGF, b-FGFand ET in the bronchial asthma airway remolding model, and decrease the expressionof VEGF, b-FGF and MVD in lung tissue, which can control and relive the airwayremolding. |
