Objective: To acquire an ideal and safe analgesia project definitive effort and seldom side-effect.Methods: Fifty-four patients of combined hemorrhoids needed to surgery were divided into three groups: group A was treated with epidural analgesia (20 cases) after operation, group B with partial analgesia (14 cases), group C without any analgesia(20 cases). Group A was with CSEA combined narcosis , whose epidural tube was linked with self-control analgesia pump( with 187. 5mg ropivacaine , 0. 3mg fentanyl and 8mg ondansetron dissolved in 100 ml sodium chloride jnjection ), after injected with 0. 1875% ropivacaine 4 ml at the end of the operation .Group B , using lumbar narcosis , was injected with 0.2% compound methylene blue injection 6-10 ml (2% methylene blue 1ml , 0. 75% bupivacaine 5ml and 2% lidocaine 4ml). Group C was also with lumbar narcosis , using no analgesia drugs. Compared the three groups with compound symptoms such as stitch, nausea and vomiting , urinary retention, the edge edema ,headache ,swirling , itch , heart-throb ,urticaria, etc, and the need of analgesia drugs as well as life-sign and the moving blockage of lower limbs in group A.RESULTS: There was significant difference in VAS scores in 2-72 hours after operation among 3 groups (0. 81±0. 41,2. 79 ± 0. 89,5. 29 ± 0. 91, P<0. 01); and after the first defecation (3. 20 ± 0. 77,4. 07 ± 0. 92,5. 80±1. 24, P<0. 01); The VAS scores in the defecation after 72 hours was 3. 43 ± 0. 68, 3. 76 ± 0. 91, 4.95 ± 1.06, and there was significant difference in VAS scores in B&C and A&C(P<0. 01), while no significant difference in A&B(P>0. 05). The number of tube drainage urine were 2 cases (group A) ,3 cases (group B), 8 cases (group C),and lighted urinary retention were 2 cases (group A) ,1 cases (group B), 2 cases (group C). The was 1 case of headache in group B, and 1 cases of nausea and vomiting in group C. There was no compound symptoms in group A, who was not seen obvious moving blockage of lower limbs, breathe control, and low blood pressure.
