Temporomandibular Joint Disorder Patients (occlusion) Contact With The Characteristics Of The Study

Temporomandibular disorders(TMD) was the name of a set of diseases which invoved in temporomandibular joint and/or masticatory muscle system .TMD was diffcult to treat because its' pathogeny was complex .There were still lots of debates with regard to if occlusal factors were one of the causes of TMD and the role of occlusal therapy in the treatment of TMD .In this paper ,the experimental reaserch was carried out to study the occlual contact characters of TMD and the effect of occlusal splint in correcting occlusal contacts characters of TMD with T-Scanll system ,so that reference could be offered to investigat the mechanism of occlusal splint.This study included two parts as follows :PartI :The study on characters of occlusal contacts of TMDObjective:To investigate the characters of occlusal contacts of TMD . Methods:The occlual contact characters of 32 cases with TMD and 30 controls were measured with T-Scanll system in the intercuspal occlusion position, the protrusive movment as well as lateral movement. Results:The elusion time , disclusion time , distance between center line and center of occlusal force of TMD group were longer than those of the control group and there are significant difference between them ; Compared with the control group ,the incidence rate of the premature contact was higher, however, the lateral and protrusive balanced occlusal contacts showed no significant difference between the two groups. Conclusion: Within the limitations of this study ,the results indicated that the premature contacts was a factor closely related to TMD, the center of occlusal force, the elusion time and disclusion time were important reference for lesion degree and therapeutic efficacy of TMD.Part II: The study of effect of occlusal splint on changing occlusalcontact charactersObjective:To compare the effect of occlusal splint on changing occlusalcontact characters and investigate the mechanism of occlusal splinttherapy.Methods: 34 TMD were treated by occlusal splint .After two month ,Thechanges of occlual contact characters and symptom were recorded andcompared with the effect before treatment.Results:The elusion time , lateral disclusion time . distance betweencenter line and center of occlusal force of TMD decreasedobviously ;Compared with the effect before and after treatment ,theincidence rate of the premature contact was lower, however, the lateral andprotrusive balanced occlusal contacts as well as protrusive disclusion timeshowed no significant difference between the two groups.Conclusion: Within the limitations of this study ,the results indicated that :there was a distinct effect of occlusal splint in correcting elusion time ,lateral disclusion time incidence rate of the premature contact as well asdistance between center line and center of occlusal force of TMD .It isimportant mechanism of occlusal splint to harmonized the occlusion ofTMD.
