Objective Lots of literatures had reported the methods that digested the model of rat's OLT were revolved in the classical Kamada's two-cuff technique of orthotopic liver transplantation in the rat. But the techniques were rather complicity, and anhepatic period was long, and the survival rate was influenced much after the surgery. We made a lot of improvements in the cuff technique. To establish an advanced animal model of two-cuff technique of orthotopic liver transplantation in the rat and conclude related operation skills. New cuff technique simplified the way of operation, shortened the operation, and increased the survival rate for rats liver transplantation. Methods Orthotopic liver transplantation was conducted in 30 rats by modified cuff technique. The anastomosis of portal vein between donor and recipient was performed by cuff technique. The same method was used to anastomose infrahepatic vena cava. The suprahepatic vena cava anastomosis was performed by suture and bile duct anastomosis was completed end to end by teflon catheter. The all operation finished by one single. The time of acquiring donor liver, repairing liver, anhepatic phase, suprahepatic vena cava's suture phase, anastomosis phases of PV,IVC and bile duct were noted. The all operations finished by myself.Results The average time of acquiring donor liver was 24.20±2.89minutes, repairing liver was 19.77±2.69 minutes, anhepatic phase was 11.33±1.42 minutes, the suprahepatic vena cava's suture phase was 8.63±1.61, the anastomosis phase of portal vein between donor and recipient was 2.70±0.75 min, the anastomosis phase of IVC was 2.87±0.78 min and bile duct was2.07±0.87min. 1 cases died after the surgery into 24h, the operative successful rate was 96.67%(1/30). 2 cases died in one week, the survival rate was 90% in one week. The main causes of death among the survived over 24 hours were obstruction and necrosis of biliary tract. Conclusion Making orthotopic liver transplantation in the rat requires professional microsurgical techniques and careful operation. The results have evident superiority in operation success rate and survival rate after OLT than the record of literatures. The improved two-cuff technique possesses advantages of short anhepatic phase, high operative successful rate, and is an ideal method for establishing this animal model.
