Hypertensive disorder complicating pregnancy is a specific disease in pregnancy, which includes gestational hypertension,pre-eclampsia,eclampsia,pre-eclampsia superimposed upon chronic hypertension.Its incidence rate is 9.4%inland and 7%~12%abroad.Its main clinical manifestation is temporary hypertension, proteinuria in pregnancy.In recent years,many scholars at home and abroad introduced the concept of early onset severe preeclampsia(ES-PE),it means that the onset period of severe pre-eclampsia is before 34 weeks in pregnancy.Because early onset severe preeclampsia has an early invasion and a fast progression,and always accompanying with more complications,which result in the earlier multiple organ dysfunction,so it is a serious threat to the safety of pregnant women and fetuses,and have a higher maternal and perinatal infant mortality.It is difficult problems of obstetric physicians.So far,the etiology of hypertensive disorder complicating pregnancy has not been illuminated clearly.There are several theories about its etiology:placenta shallow invasion,vascular endothelial damage,genetic factor,alimentary deficiency, insulin resistance and so on.Nuclear factor kappa B(NF-κB) are found in recent years,a nuclear transcription factor,and its activation to participate in and start regulating the immune and inflammatory responses,apoptosis and cell proliferation and differentiation related gene transcription.Fibroblast growth factor(FGF) are the early 70s from cow brain and pituitary in the purification of a polypeptide growth factor for acidic FGF(aFGF) and basic FGF(bFGF).bFGF on endothelial cells,mainly through the direct role of mitogen to promote angiogenesis,in vivo bFGF can induce vascular endothelial cell proliferation,migration and angiogenesis.Platelet-derived growth factor(PDGF) are generated by a variety of cells,peptide growth factor,at physiological conditions at the main storage of a platelet particles in a variety of blood cells in vivo,tissue cells and certain tumor cells may produce and secrete PDGF,mainly by megakaryocyte synthesis,in tissue repair,embryonic development,immune response,tumor cell proliferation plays an important role.This article on the NF-κB,bFGF and PDGF with the early onset severe preeclampsia were studied.ObjectiveThis study was designed to investigate and compare the expression of NF-κB, bFGF and PDGF in placental villus of normal pregnancy,early onset severe pereclampsia and late onset severe preeclampsia,using immuno-histochemistry and image analysis.This study will help to illuminate the precess of pathphysiology of severe pre-eclampsia,to supply some theoretic foundation about regulation of trophoblast invasion and to supply some new methods about diagnosis,prevention, prevention,treatment of preeclampsia.Materials and Methods1.Object of research:57 cases of pregnant women from Oct 2007 to April 2008 who admitted to the Third Affiliated Hospital of Zhengzhou University,aged 22-37 years old and gestated 32-37 weeks.All the pregnant women were divided into three groups:①early onset severe preeclampsia group:19 cases,the average age is(31.61±3.13) years,gestational age of delivery is(34.78±2.07)weeks;②late onset severe preeclampsia group:18 cases,the average age is(30.47±2.84)years,gestational age of delivery is(35.77±0.79)weeks;③normal pregnancy control group(no obstetric and pregnancy complications,no hypertension and hypertensive disorders complicating pregnancy family history) 20 cases,the average age is(29.85±2.89)years,gestational age of delivery is(36.04±1.42)weeks. diagnostic standard:Obstetrics and Gynecology edited by Feng Youji,Shen Keng.Three groups of pregnant women were all single births,cesarean section delivery,and with on pregnancy complications or other medical complications.The maternal age and gestational age have no statistical differences.2.Method:The placenta of each group was stained by HE,all samples were used for studying expression and quantification of NF-κB,bFGF and PDGF by immunohistochemistry staining and image analysis.All experimental data were processed by SPSS 13.0 and statistical significance was defined as P<0.05.Results1.General information comparison:the women's age and the delivery age of normal pregnancy and severely early-onset and late onset pre-eclampsia groups have no statistical differences(P>0.05).2.Intensity of NF-κB immunostaining in trophoblast was significantly higher in the severely early-onset pre-eclampsia group compared with that in the normal controls;Expression of NF-κB in the late onset severe pre-eclampsia group was significantly higher than that in the normal group,too(P<0.05).But there was no significant difference in immunostaining for NF-κB between the early onset severe pre-eclampsia and the late onset severe pre-eclampsia groups(P>0.05).3.Intensity of PDGF immunostaining in trophoblast was significantly higher in the severely early-onset pre-eclampsia group compared with that in the normal controls;Expression of PDGF in the late onset severe pre-eclampsia group was significantly higher than that in the normal group,too(P<0.05).But there was no significant difference in immunostaining for PDGF between the early onset severe pre-eclampsia and the late onset severe pre-eclampsia groups(P>0.05).4.The expression levels bFGF in three groups:The expression level of bFGF in early-onset and late-onset severe pre-eclampsia group were lower than normal pregnancy group,the differences have statistically significance(P<0.05);early-onset group lower than late-onset group,the differences has statistical significance (P<0.05). 5.Pearson correlation analysis:there was positive correlation between NF-κB and PDGF(r=0.316,P=0.017)Conclusion1.The expression high level of NF-κB,PDGF and low level of bFGF in early onset severe pre-eclampsia group.These results indicated that NF-κB,PDGF and bFGF may influence pathogenesis of the early onset severe pre-eclampsia by some mechanism.2.We maybe conclude that there are differences between early onset severe preeclampsia and late onset severe preeclampsia from this study.The author believed that further study may help us to find more wants. |
