A Clinical Comparative Study On Four Kinds Operations To Treat Adult Inguinal Hernia

BackgroundInguinal hernia is a common and multiple disease. If you want to cure inguinal hernias, the only choice is surgical repair. Nowadays, there were many a kind of inguinal herniorrhaphy. Surgeons have different suggestions and arguments about how to choose the surgical methods for inguinal hernia.ObjectiveThe objective of this thesis was to contrastively analyse the surgery effect of bassini inguinal herniorrhaphy, Lichtenstein tension-free hernia repair(Lichtenstein tension-free herniorrhaphy), plug-mesh tension-free hernia repair (Rutkow's operation) and transabdominal preperitoneal approach (TAPP) and direct clinicians to choose the best proposal of the operation.MethodsFrom January 2005 to December 2009,369 patients with inguinal hernia underwent hernia repair in the Naikai Hospital of Tianjin and relative materials were retrospectively analyzed. All patients above received surgical treatment and obtained follow up. We had analyzed comparative some clinical indexes of four groups (group bassini, group Lichtenstein, group Rutkow, group TAPP) cases.t-test was used to analyze the quantity data and test was used to analyze the quality data by the SPSS 13.0 statistic soft ware pack.ResultAverage length of stay of group TAPP were 5.1 days,less obviously than other three groups(p<0.05).The cost of hospitalization of group TAPP was 16387.7 yuan, more than other three groups (p<0.05).between group TAPP and group bassini,the postoperative venting time of the former earlier than the later (p<0.05). The time of operation, the highest postoperative temperature and the emergence time of the highest postoperative temperature of four groups have no statistics meanings. The total incidence rate of complication of bassini group is 16.36%(9/55), Lichtenstein group is 9.80%(5/51), Rutkow group is 11.95%(27/226), TAPP group is 10.00 %(2/20).The recurrence rate of bassini group is 3.64%(2/55), Lichtenstein group is 1.96%(1/51); Rutkow group is 0.88%(2/226); TAPP group is 0。The total incidence rate of complication and the recurrence rate of bassini group higher than other groups (p<0.05)。The difference of the other indexes have no statistics meanings. (p>0.05)。ConclusionsTension-free hernioplasty have some advantage to compare with traditional herniorrhaphy, such as small trauma, few complication and low recurrence rate. We can choose the most suitable herniorrhaphy,when we control the characteristic of various sorts and varieties herniorrhaphy and concrete pathogenetic conditions.
