[Objective] To study the singnificance of high-mobility group box chromosomal protein 1 (HMGB-1) in severe acute pancreatitis, explore the protective effect somatostatin combined with growth hormone on SAP. HMGB-1 provide a new evaluation index for clinical therapy in SAP patient.[Methods] Male SD rats were randomly divided into six groups, each group 40 rats:sham operation group; SAP group; SAP treated by normal saline group; SAP treated by somatostatin group; SAP treated by growth hormone group; SAP treated by somatostatin combined with growth hormones group. Retrograde pancreatic-bile duct injection of 3.5% sodium taurocholate 2.5ml/kg, keeping the pressure 3.4-3.9kPa, was used to establish SAP rat models. Concentrations of endotoxin,IL-1β,IL-6,TNF-αand expressions of pancreatic tissue HMGB-1 mRNA were measured in each group respectively. Gut histopathological changes were also observed. Pancreatic pathological evaluation graded with Schmidt criterion was also performed. Clincal date from 2005-6 to 2009-12 inhospital SAP patients, Divided into somatostain group(n=11) and somatostatin combined with Growth hormones group(n=10), and then analyse the relationship HMGB-1 With SAP complicated with MODS and bacterium translocation;evaluate The effect to HMGB-1 treated by somatostatin combined with growth Hormones.[Results] HMGB-1mRNA were delayed overexpressed in SAP rats. In SAP rats treated by somatostatin combined growth hormone, serum Endotoxin,TNF-α,IL-6 and IL-1βdecrease, the inflammatory reaction in pancreatic tissue allcviate, as well as pancreatic HMGB-1mRNA expression. Somatostatin combined with growth hormone also can relieve MODS and bacterial translocation in SAP patients as well as decrease serum HMGB-1 level.[Conclusion] The level of pancreatic HMGB-1mRNA in SAP was the same Tendency with the serum endotoxin,TNFα,IL-6,IL-1 and pancreatic pathological evaluation. Serum HMGB-1 was ascended in SPA patients complicated with MODS and bacterial translocation, after treated by somatostatin combined with growth hormone decreased.
