Objective:To assess the value of the Sequential Organ Failure Assessment (SOFA) score in evaluating the prognosis of patients receiving continuous renal replacement therapy (CRRT) after cardiac surgery.Methods:From 2004 to 2013,92 patients with acute kidney injury (AKI) after cardiac surgery who received CRRT for more than 48h in our hospital were evaluated. Organ dysfunctions were evaluated by SOFA score (excluding renal points) on the first and third days of CRRT. We also calculated the absolute (A-SOFA) and relative (A-SOFA) changes in SOFA scores. According to A-SOFA, the patients were divided into three groups:improved group, unchanged group and worsened group.we compared the mortality among 3 groups. And then we divide these patients into two groups according to the result of therapy:survivors and non-survivors. Also, we compare the difference between these two groups. The risk facts of mortality were also assessed by Logistic multivariate regression analysis.Results:The overall mortality is 54.3%. Group of organ dysfunction improved(A - SOFA< 0) consists 27 cases, Group of organ dysfunction remaining unchanged (A-SOFA=0) consists 23 cases, Group of organ dysfunction worsened(A-SOFA> 0) consists 42 cases. The mortality rate of improved group is obviously lower than the rest two groups (25.9%、65.2%、66.7%, p=0.002).All of SOFA score on third days, A-SOFA and △-SOFA in group of non-survivor are higher than the survivors.Conclusions:For patients in need of CRRT after cardiac surgery, early changes in SOFA score can predict the prognosis.
