Objective:To investigate the clinical characteristics,cell origin, and prognosis of (primary gastric diffuse large B-cell lymphoma (PG-DLBCL), and explore effective treatment parrerns and prognostic facters of patients for PG-DLBCL. Method:Clinical data of 82 PG-DLBCL patients , diagnosed and treated in The First Affiliated Hospital of Nanchang University from 2001 to 2009, were retrospectively analyzed.Survival rate was by Kaplan-Meier method, prognosis factors were analyzed by log-rank test,and Cox model multivariate ananlysis.Forty-four of 82 patients received surgery plus chemotherapy,5 received surgery plus chemoradiotherapy,8 received chemoradiotherapy,8 received surgery alone,15 received chemotherapy alone,and 2 were untreated.Results: The median age of the 82 patients was 54(18~80) years .The ratio of sex ( male : female ) was 1.05∶1. Among the 72 patients who had been followed up , 22 ( 30.6%) died , the 3- and 5-year overall survival rates were 64.2% and 59.3%,respectively. The complete remission rate(CR) of Surgery plus chemotherapy or chemoradiotherapy group and chemotherapy or chemoradiotherapy group were 93.9% and 78.3%(p=0.1), respectively.Log-rank test revealed that International Prognos tic Index ( IPI) score, B symptoma andβ2-microglobulin level were important prognostic factors of PG-DLBCL. Cox model multivariable analysis indicated that the serum level of LDH and clinical stage of the patients at diagnosis were independent prognostic factors for PG-DLBCL patients.Conclusions:PG-DLBCL is often seen in male patients at middle ages. IPI score,B symptom andβ2-microglobulin level are important prognostic factors of PG-DLBCL. The serum level of LDH and clinical stage of the patients may be independent prognostic factors. CR of Surgery plus chemotherapy or chemoradiotherapy group is higher than that of chemotherapy or chemoradiotherapy group.
