Objective: To explore the changes of serum T cell subsets (including CD3~+,CD3~+CD4~+,CD3~+CD8~+,CD4~+/CD8~+) and immunoglobulin(including IgA,IgG,IgM) levels in serum and induced sputum in asthmatics under the extreme weather conditions, and discuss the variation of the whole body and airway cellular and humoral immunity on patients with asthma. Methods: Collecting 40 patients with mild to moderate asthma(20 cases were divided into Seretide group and 20 cases into BCG-PSN group randomly, patients in Seretide group took 50/250ug Seretide alone, while patients in BCG-PSN group took both 50/250ug Seretide and BCG-PSN), collecting 4ml venous blood and 1.5ml induced sputum at three observation points(Baseline point, Sudden temperature drop point, very cold temperature point) respectively, using FCW and ELISA to measure the level of CD3~+,CD3~+CD4~+,CD3~+CD8~+ and CD4~+/CD8~+ ratio in plasma and the level of IgA,IgG and IgM in serum and induced sputum, to evaluate the systemic and local immunity in asthmatics under extreme weather conditions. Results: In both two groups, compared with the baseline point and very cold temperature point, the level of CD3~+ and CD8~+ in plasma decreased significantly (P<0.01), the level of CD4~+and CD4~+/CD8~+ increased significantly (P<0.01) and the level of IgA,IgG and IgM in serum and induced sputum decreased significantly (P<0.01) at sudden temperature drop point. Compared to the baseline point, the level of IgG in induced sputum decreased at very cold temperature point in two groups(P<0.05), there had no differences of the level of IgA and IgM in induced sputum at the baseline point and very cold temperature point between two groups; At s udden temperature drop point, compared with Seretide group, the level of CD3~+and CD8~+ in plasma increased significantly (P<0.01), the level of CD4~+and CD4~+/CD8~+ decreased significantly (P<0.01) and the level of IgA,IgG and IgM in serum and induced sputum increased significantly (P<0.01) in BCG-PSN group. Conclusion: The cellular and humoral immunity of patients with asthma were influenced much more at sudden temperature drop point than that at very cold temperature point. Based on the use of Seretide plus BCG-PSN can enhance the immunity of patients with asthma. It showed some significances for decreasing the morbiity of asthma induced by the change of systemic immunity due to a sudden temperature drop. The study can provide some references for preventing and treating asthma under extreme weather conditions.
