[Objective] To survey the feasibility of developing donation after cardiac death (DCD) in Tianjin, one of the selected experimental cities. Then based on that, to provide concrete data support for carrying out investigation on hospitals throughout the city and establishing the potential donor pool for DCD.[Methods] (1) The diseases constitute of inpatient death cases in Tianjin First Center Hospital between 2007 and 2009, was analysed by retrospective study based on the "International Classification of Diseases" ICD-10 disease classification principle.(2) The general information, function of important organs of accident trauma, injury and poisoning and cerebral vascular accident deaths were analysed, and taken them as the potential DCD was evaluated (3) The ralatives of the fatalities in accident trauma, cerebral vascular accident were investigated by questionnaire on their organ transplant awareness, attitudes and will of organ donation.[Results] (1) Through the analysis of death cause of 2,702 cases in Tianjin First Center Hospital between 2007 and 2009, the largest proportion was cancer, accounting for 24.32% of all the deaths, followed by respiratory diseases (20.32%), circulatory system diseases (18.84%), injury and poisoning (9.88%) and neurological diseases (9.84%).267 cases died of injury, poisoning and accidental which were mainly the young people aged 15 to 44 years group (accounting for 48.69%). They were mainly due to car accidents caused (accounting for 65.92%). (2) 117 cases met the clinical standard of liver and kidney donation through the comprehensive assessment of 127 cases died of traffic accident injury, cerebral vascular accident, accounting for 4.33% of total number of cases. (3) The investigation of the families having deaths from car accident trauma, injury and poisoning, and cerebrovascular accident showed that most people (51.33%) had never heard of the organ transplantation,33.33% of the people had the donated will themselves.36.67% of them disagreed with family members'donation, but 42.00% of them trend to making the decision of donation after their family members' dissusion.[Conclusion] The prevention and treatment of the top five diseases by cause of death should be reinforced to reduce the fatality rate and improve the quality of life. In order to maintain the homeostasis of the potential donor of DCD, such as the stabilization of haemodynamics and the function of endocrine system, the medical workers should optimize the correlative examinations during admitting and treating the patients with critical illness. This would benefit for increasing the number of potential donors. The public still had much vacancy on understanding and cognizing to the organ donation. The correlated departments should strengthen the dynamism and broad the extent of social propaganda on the organ donation. They should permeate its affirmative meaning into the schooling to make the public recognize its significance gradually. So that, we might sufficiently discover more potential postulants for organ donation. There still has long way to go and need extensive support to it.
