To evaluate the relationship and role between the nuclear factor-KB and matrix metalloproteinases-9 in the pathogenesis of intracranial aneurysms development and enlargement.Methods:Thirty surgical specimens of human cerebal aneurysm from 13 males and 17 femals in Sichuan Provincial People's Hospital during 2009.6-2010.2 were choosen. All specimens were got from dissecting after clipping the aneurysm in craniotomy with the guarantee of protecting patient and operation safety. Twelve control specimens included 4 cases of suferficial artery,4 cortex artery,2 anterior cerebral artery and 2 middle cerebral artery. These specimens came from hematoma removal and internal decompression. To observe the all specimens with macroscopic and light microscope. To detect the expression of NF-κB and MMP-9 in human cerebral aneurysm with immunohistochemical method.Results:1 macroscopic observation Most of the specimens were purple, dark blue or brown.Atherosclerosis plaques could be seen in some of the aneurismal specimens.There were thrombosis in some cavities of the aneurysms. The wall of the aneurysm varied, the bottom was thicker than the top.Sometimes, only a thin membrane at the top of the aneurysm was left, some were ruptured.2. H&E staining:The fundamental pathological change in cerebal aneurysm wall was the number of endothelial cell decrease; disintegration of internal of elastic layer;VSMCs were scarce in the tunica media and dispalced by fiber tissue; the outer layer turned thinner, most was fibre tissue, no outer elastic layer. In all layers of aneurysm were coated with more or less inflammation cells, regional calcification,thrombosis and different stage of atherosclerosis in the aneurysm wall.3. Immunohistochemistry stain:The positive expression of NF-κB and MMP-9 in all samples every layer in the wall of aneurysm. NF-κB was mainly expressed in the inner and middle layer of aneurysm and endothelial cell, smooth muscle cell and macrophage cell. Positive expression of NF-κB in the wall of intracranial aneurysm shows a claybank staining of the nucleus and cytoplasm.MMP-9 were mainly expressed in the adventitia and middle layer of aneurysm and endothelial cell, smooth muscle cell and macrophage cell. Positive expression of MMP-9 in the wall of intracranial aneurysm shows a claybank staining of the cytoplasm. In aneurysm walls shows positive rate of expression of NF-κB at 83.33%(25/30) and expression of MMP-9 at 80%(24/30). While in the control vessel shows negative rate of expression of NF-κB at 8.33%(1/12) and expression of MMP-9 at 16.67%(2/12).Conclusions:1.The morphological and pathological characters of human intracranial aneurysm were inflammation and its destruction of the aneurismal wall.The main inflammatory cell were macrophage and lymphatic cells.2. The positive expression of NF-κB and MMP-9 in expression every layer in the wall of aneurysm.
