| Objective To investigate and analysis the single segmental cervical physiological height and curvature on cervical spondylotic patients with receiving three different types anterior of cervical discectomy and fusion. Methods In the retrospective study, Among forty cases (32 males and 18 females) of Cervical spondylosis were divided into three groups and were followed up in the Sichuang Provincal People's Hospital from 2007 to 2009,31 cases were subjected to decompression, titanium net bone graft pedestal to fuse internal fixation (group A), the remaining 13 cases is Cage (group B) and 6 cases is cervical disc replacement (group C) respectively. Radiological measurements were performed on the cervical radiographs taken before operation, one week,3-6 months and 12 months after operation, the physiological height and physiological curvature of the fused segment, functional restoration, and clinical outcome were evaluated in the three groups. Results The follow-up showed that the anterior cervical heights were (4.13±6.31) mm, (3.85±0.98) mm and (3.89±1.29) mm in the group A,B and C in 1 week after operation respectively, with the insignificance among the three groups (χ2=2.751 P>0.05). The posterior heights were (4.10±6.53) mm, (3.88±1.05) mm and (3.82±0.79) mm, with the insignificance among the three groups (χ2=2.833 P>0.05). During postoperative 3-6months and 12 months followed up, group A lost gradually and group B and C maintained well. The intersegmental curvature at the surgical site were (7.80±4.48) mm, (8.70±2.23) mm and (10.84±0.82) mm in the group A,B and C in 1 week after operation respectively, with a significant differences among the three group respectively (χ2=5.356. P<0.05). The curvature were (7.44±2.21) mm, (8.53±1.23) mm and (10.58±0.75) mm in 12 months after operation respectively, with a significant differences among the three group respectively (χ2=3.032. P<0.001). Group B and C is batter maintained preferably than group A. There were no significant differences of the function assessment among the three group(P>0.05).Conclusion The fused segment for the single segmental cervical physiological curvature and height had recovered and maintained by the three different anterior surgeries, which at the physiological curvature by the cervical disc replacement is better than the titanium net bone graft pedestal and Cage. The clinical symptoms of single segmental cervical spondylosis have improved with varying degrees receiving different operative anterior surgery, no difference is the function assessment. |
