Analysis Of Microsurgical Treatment For Gustilo â…¢ Of Tibial Fractures

AIM:To Through two methods of surgical treatment efficacy analysis, to explore different factors that caused the type of Gustiloâ…¢Tibial Fractures's therapeutic method, and analyze the impact Gustiloâ…¢Tibial Fractures prognostic factors, to enhance efficacy and provide a theoretical basis for the improvement of the prognosis in patients.Methods: Retrospectively analyzed 60 patients with Gustiloâ…¢Tibial Fractures of the medical records and complete clinical and follow-up data, according the standard"Anderson-Gustilo"classification standard to typing and summing complication for comparison of efficacy. Injured extremity of the 60 cases were treated by emergency debridement blood vessels, nerves and tendons, soft tissue surgery suture or skin flap coverage the effective fracture fixation(A); double or multiple debridement blood vessels, nerves and tendons, soft tissue surgery suture or skin flap coverage external fixator combined with limited internal fixation(B).Results: 60 cases in this group can be devided into: 38 cases ofâ…¢B-type, 22 cases ofâ…¢C-type, of which 31 cases (51.7%) complicated with shock, 3 cases (6.7%) of bone compartment syndrome; vascular-anastomosis in 47 cases, superficial infection in 9 cases, osteomyelitis in 4 cases, vascular crisis in 4 cases, amputation in 4 cases,all patients were followed up 6 to 30 months, an average of 18 months, and according with the standard Injured extremity Puno postoperative functional assessment: excellent in 21 cases, good in 16 cases, 11 cases may be poor in 12 cases, excellent rate of 61.7%.Conclusions: Gustiloâ…¢Tibial Fractures orthopedic surgeon for clinical treatment of challenging, however, the advance of this micro-cycle dynamics reconstruction techniques, which including limb functional reconstruction, complex wound tissue coverage, increased the opportunities to save the limb. The key to the treatment of limb Gustiloâ…¢type fractures ies in early soft tissue coverage, cycle power function of the reconstruction, and stable fracture fixation.
