Objective: Acute myocardial infarction(AMI) is one of the common emergent and severe cases in department of internal medicine with high lethality rate and mutilation rate, which makes more youthful and costs high. As research with large sample in every domain about patients with AMI in Chongqing is not common, we follow up the patients with AMI for long-term. We recorded and analyzed the information of the patients for their treatment, education, drug administration and also their diet, so we can provide feasible treatment of health and therapy to improve their life quality and survival rate.Methods: We reviewed and analyzed the data of 578 patients with AMI from National three level of first-class hospitals in Chongqing during Jan 2001 and Jan 2005. The data of the cases included the general conditions, medical history, medical examination, treatment, drug administration. At the same time we finished a five-year follow-up study. We acquired and analyzed the extramural hospital treatment, diet and life quality. At last, 522 cases were selected(non-cardiogenic deaths and people lost to follow-up were precluded). Results: The cumulative cardiogenic deaths was 164. Five-year mortality was 31.4%. The main characters of deaths included elder age, smoking, mental labourer, low LVEF, high Killip classification, anterior myocardial infarction, hypertension, diabetes mellitus and hyperlipidemia; more shock and arrhythmia onset, less treatment with PCI, anticoagulant,β-blocker and ACEI/ARB, the poor compliance of extramural hospital treatment. Logistic regression analysis demonstrated that elder age, hypertension, smoking, low LVEF, less treatment with PCI and the poor compliance of extramural hospital treatment were independent risk factors of five-year mortality in AMI patients.Conclusion: 1. The patients with AMI almost died in 1 year after discharge from hospital in Chongqing, the annual death rate is 19.5%.2. Compliance of extramural hospital treatment in AMI in Chongqing area is very poor. There is still existed gap between guidelines and clinical practice.3. Elder age, hypertension, smoking, low LVEF, less treatment with PCI and the poor compliance of extramural hospital treatment were the independent risk factors of five-year mortality in AMI patients.
