| Objective:To investigate the Correlation of plasma FGF21 level with obesity and insulin resistance (IR) in nonalcoholic fatty liver patients.Method:Depending on complicated with or without type 2 diabetes, the 70 cases of NAFLD patients were divided into fatty liver (FL,36 cases) group and type 2 diabetes complicated with fatty liver (DFL , 34 cases)group;There are 26 subjects in the normal control (NC) group. The plasma FGF21 level of the subjects in those groups were determined by enzyme-linked immunosorbent assay method; At the same time, fasting plasma glucose (FPG), fasting insulin(FINS), free fatty acid(FFA), lipid levels were also tested, and measurement of height, weight and WHR were implemented; The state of insulin resistance was evaluated using steady-state model HOMA-IR and insulin sensitivity index (ISI).Results:(1) In both of two fatty liver groups, the proportion of obesity, BMI and WHR were all obviously higher than those in the NC group, triglyceride (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), FFA were significantly upgraded, high-density lipoprotein cholesterol (HDL-C) downgraded; HAMO-IR in T2DM complicated with fatty liver group was obviously higher, and ISI notably reduced;(2) Plasma FGF21 levels of patients in fatty liver and T2DM complicated with fatty liver groups were significantly higher than those in the control group [ (1.86±0.64) and (1.97±0.60) ng / ml vs (1.26±0.43) ng / ml, respectively ], P <0.05 for both;(3) Correlation analysis showed that, plasma FGF21 levels was positively correlated with BMI, WHR, HOMA-IR, TG, FFA, and was negatively correlated with HDL, ISI; Multiple stepwise regression analysis indicated that WHR, HOMA-IR and TG are independent relevant factors of serum FGF21 level.Conclusion: Plasma levels of FGF21 is elevated in NAFLD patients, FGF21 closely related with obesity, insulin resistance and lipid metabolism disorder; It is believed that FGF21 has seem played a role in the occurrence of fatty liver and its development. |
