Immunoglobulin A nephropathy (IgAN) is a form of mesangial proliferative nephritis,the defining hallmark of IgA nephropathy is deposition of IgA or IgA-dominant immune complex in the glomerular mesangium accompanied by a mesangial proliferation. IgAN is the most common form of primary glomerulonephritis around the world, and it is the most common cause of ESRD( end-stage renal disease) in China. A retrospective survey showed that the frequency of IgAN increased these years. The degree of histopathologic injury of IgAN is extremely variable, and this is reflected in the varied severity of clinical presentation seen in this disease.The pathogenesis remains obscure and mysterious, therefore the patient can't be treated properly to aim directly at the etiological factors.The immunopathogenesis is a charming breakthrough point of IgAN,much progress has been made in the recent years, but many of the pathogenesis are controversy and still on the level of hypothesis, for example the mechanisms involved in formation of aberrant glycosation IgA1 and the it's process of deposition in the glomerular mesangium; the initiation of inflammatory glomerular injury; the role of the mucosa-bone marrow axis played in the development of IgAN. This review will describe the progression of immunopathogenesis of IgAN. |
