Clinical Analysis Of Different Operation Ways For Hepatolithiasis Complicated Extrahepatic Bile Duct Calculus

Objective: To explore rational and effective operation strategies by comparing early complication rate and lately complication rate of different operation methods(draininge T– tube,Roux - y anastomosis,hepatectomy adding draininge T– tube)for hepatolithiasis complicated extrahepatic bile duct calculus.Methods: Three hundred and fifteen cases of hepatolithiasis complicated extrahepatic bile duct calculus treated in our hospital from January 2004 to December 2009 were analyzed retr ospectively.Three operation methods(draininge T-tube, Rouxy anastomosis, heap tectomy adding draininge T– tube) were compared in respect of early complications and long - term outcomes. Follow- up period was 1 -6 years. Results: The postoperative stone residual rate of the group which was underwent hepatectomy adding draininge T– tube was 14.3%,and it was obviously lower than that of draininge T– tube and Roux - y anastomosis(P<0.05),The other early complication rate had no diffference(P>0.05). Calculus relapse rate and cholangitis rate and reoperation rate in Roux - y anastomosis and draininge T– tube patients was obviously higher than in hepatectomy adding draininge T– tube patient(P<0.05). Conclusion: The outcome of hepatectomy adding draininge T– tube is superior to that of draininge T-tube and Roux-y anastomosis .The function of Oddis'sphincter is remained and hypoechoic focal lesion in liver is removed in patient underwent hepatectomy adding draininge T.The use of hepatectomy adding draininge T is beneficial to decreasing the postoperative stone residual rate,calculus relapse rate,cholangitis rate,reoperation rate.
