Clinical Observation On Cuative Effect Of Xuebijing Injection In Curing Chronic Obstructive Pulmonary Disease With Acute Exacerbation And Its Effect On Endotoxin

[Objective]Through the exploration of AECOPD patient's characteristics of TCM hou,then definitude furtherly distribution rule of AECOPD patients'syndrome.Evaluating the clinical efficacy of Xuebijing injection treating acute exacerbation of chronic obstructive pulmonary disease,and the effects on the level of bacterial endotoxin, blood gas analysis, and D-dimer,etc.[Method]firstly, on the basis of summarizing the clinical experiences of elder physician and related literature, preliminaryly identified 13 types of common TCM hou for AECOPD and the diagnostic criteria.According to the croteria and expert advice, formulated a TCM hou watchlist,collected 124 AECOPD patients,through the TCM four diagnostic methods,recorded each paitent's symptoms and signs,then identified each TCM hou. At last,Statistic and analysed.Secondly,according to the diagnostic and include criteria,53 AECOPD patients(qi deficiency phlegm-heat blood stasis syndrome) were recruited from February 2009 to February 2010 from our inpatients,and divided into two groups,27 cases as controlled group,which were treated with conventional therapy of western medicine,26cases as treated group,which were treated with xuebijing injection and conventional therapy of western medicine. All patients were given conventional therapy of western medicine included controllable oxygen treatment,anti-infection,dcissipating,spasmolysis,antasthmatic,and ventilation support,while the treated group were added xuebijing injection 50ml,bid X 7 days.The syndrome and objective sign of all paitents were recorded and scored,the blood gas analysis,D-dimer,WBC and N% were detected on the day before treating and the first day after treating. Endotoxin was detected at 7:00 in the morning on the first,fourth,eighth day during the sutdy. At last,all data were analyzed by SPSS 13.0 for windows.[Result]1.The main excess syndrome appear in the aggravation period are blood stasis syndrome, Phlegm-heat retention in the lung,water invade the lung and heart syndrome, exterior cold but phlegmheat in lung syndrome, accumulation of phlegm dampness in the lung syndrome,invation of the lung by wind-heat syndrome, invation of the lung by wind-cold syndrome;The main deficiency syndrome are lung qi deficiency syndrome, spleen qi deficiency syndrome, kidney yang deficiency syndrome,lung yin deficiency syndrome, kidney yin deficiency syndrome.2.The treated group was superior to the controlled group in the curative effect with Significant difference (p<0.05).This indicated that the xuebijing injection can improve the total therapeutic efficacy of conventional therapy in curing AECOPD.significant differences3.In terms of relieving the primary ryndrome such as cough,sputum,there wsa no significant differences between the two groups (p<0.05).This indicated that the xuebijing injection can not relieve the primary syndrome and objective sign.4.After the treatment,The blood gas analysis was improved significantly in both groups(p<0.05),there was no significant differences between the two groups(p>0.05).There was no difference in improving the blood gas analysis between the two therapys,it indicated that xuebijing injection can not improve blood gas analysis.5.After the treatment, both of the two groups reduced the C-reactive protein significantly (p<0.05),and there was significant differences between two groups(p<0.05).6.Endotoxin in both of groups reduced(p<0.01)after the treatment,showing significant.And endotoxin reduced more greatly in the treatmed group (p<0.01).After the treatment,D-dimer in the blood of the two groups reduced(p<0.05),and there was significant differences between two groups(p<0.05).So we can see that the effect of the treated group on endotoxin and D-dimer is more significant than the control group.7.Through the whole trial there was no serious adverse effects in both groups,which indicated that it was safe to use xuebijing injection in curing AECOPD beside conventional therapy.[Conclution]1.Most of AECOPD patients exist Lung Qi deficiency and blood stasis syndrome,and patients always manifested as multi-syndrome concomitance.2.Xuebijing injection cooperated with conventional therapy of western medicine treating AECOPD can improve the total therapeutic efficacy.3.It works of the xuebijing injection in reducing the level of endotoxin.
