Clinical Study On Treatment Of Conservating Ovarian Function After Endometriotic Ovary-cyst Surgery By Invigorating The Kidney Plus Blood-Stasis-Resolving

Endometriosis is one of the gynecological disease problems,occuring on many child-bearing women.It has the property of being aggressive,extensively planted and easy to recurrence.It mostly occcurs on the ovary,which calls endometriotic ovary-cyst(Chocolate cyst of ovary). The treatment for reproductive age women mostly is conservative or semi-conservative surgery. The classic treatment of endometriotic ovary-cyst is cystectomy combined with GnRH-a.That the ovarian function of endometriotic patients usually decreases has confirmed.And endometriotic ovarian cystectomy heavily damages the ovarian function.The treatment of using GnRH-a after the surgery greatly inhibit the residual lesion,reducing the relapse rate,but also influence the growth of oocytes,which reducing the pregnancy rate.It is significant that has a clinic study on protecting ovarian function of child-bearing women,especially those eager to pregnant.Objective:This study observates the OEMA patients using GnRH-a after surgery.It explorates the Chinese traditional medicine theory of nourishing kidney with Yin and Yang plus Blood-Stasis-Resovling on conservating the ovarian function.Method:Selected were all in March 2009～March 2010 in Jiangsu Provincial Hospital of Chinese treatment of reproductive out-patient clinics, gynecological ward who have concenceived surgical treatment because of ovarian endometriosis, a total of 43 cases.It is individed into two groups,the control group using GnRH-a and the study group using Chinese medicine combinded with GnRH-a.We observe and record the syptoms'score before and after treatment.We take serum bFSH, bE2 level of early follicular phase,and measure the ovarian volume and follices with B-ultrasound machine before and after the treatment respetively. We observe the days of menstruation recover, the dominant follicle rate, the ovulation rate and BBT situation after withdrawal.Results:(1)Both two groups of patients improve obviously the ovarian function after treatment.In study group,the level of serum bFSH,bE2 and AFC after treatment obviousely better than that before the treatment (P<0.05).The level of MOV never change obviousely between pre-post treatment(P>0.05).In control group,the level of serum bFSH after treatment obviousely better than that before the treatment (P<0.05).The levels of bE2,AFC and MOV never change obviousely between pre-post treatment(P>0.05).(2) The levels of bE2 in study group decrease lower than in the western medicine group(P<0.05).The AFC of study group increased more than that of control group.The days of menstruation recover,the dominant follicle rate, the ovulation rate and the high temprature days of study group is higher than control group(P<0.05).The preliminary clinical trial shows that Chinese medicine combined with GnRH-a has a significant effects on the improvement of ovarian function.Conclusion:The ovarian function mostly decreases after the endometriotic cystectomy. The method of traditional Chinese medicine combined with GnRH-a improve the ovarian function better, while it will not influnce the function of GnRH-a to inhibit the residual focus.
