| With social aging becoming fast and drug-resistant pathogens being rampant at present, community acquired pneumonia (CAP), a common infectious disease, could be put under the categories of pyretic pulmonary disease, wind warm disorder, and winter warm disorder. Etiology and pathogenesis, syndrome differentiation of CAP and clinical application of benefiting qi and nourishing yin are discussed from the aspects of the theory research and clinical observation, which can explore treatment methods for quite a few of pulmonary infectious diseases including pneumonia.Abuse of antibiotics, glucocorticoid and antiviral drugs, unscientific medication concept, sociodemographic structure and constitution change make fever, pulse, auscultation, laborotary examination and other positive symptoms not typical as usual. CAP is usually developed in the elderly, the weak or those with chronic diseases. Deficiency of qi and yin is a significant intrinsic factor and exopathogens are external causes. When deficient lung qi, or deficient lung yin, or deficient lung and spleen qi exsits, wind, cold, summer heat and dampness invade the body, causing failure of lung qi in dispersion and then simultaneous presentation of heat, phlegm and stagnation. Phelgm, heat and stagnation are pathological production and pathogenic facors as well. Heat can cause phlegm and stagnation. Phlegm can cause heat accumulation and stagnation production. Phlegm and stagnation influence eath other. Stagnation can also make heat accumulate. So the pathogenesis is failure of lung qi in dispersion and simultaneous presentation of sthenia and asthenia. According to the physiological nature of the lung, the features of warm pathogens and transmission law and relevant clinical and pharmacologic literature, benefiting qi and nourishing yin is considered as a significant treatment principle of CAP. Nourishing yin method can promote body fluid, reduce heat and relieve superficies, and in the other aspect, invigorating qi method can strengthen healthy qi and transform yang to yin. In treatment, especially when treating the eldery with prolonged diseases or other diseases, those with repeated respiratory infection and the weak young with congenital diseases, etiological factors of CAP and the excess and deficiency of constitution must be taken into consideration. The features of fever, cough, phlegm, thirst, the tongue and the pulse of syndrome of deficiency of qi and yin should be grasped and the existence of deficieny of qi and yin should be recognized accurately in time. Then the treatment principle of benefiting qi and nourishing yin is early used rationally. Meantime, drugs of invigorating blood, clearing heat, dissipating phlegm and purging are applied accordingly.60 cases of conformed syndrome of deficiency of qi and yin of CAP were randomly divided into a control group (30 cases), treated with general treatment and anti-infective therapy, and a treatment group (30 cases), treated with Benefiting-Qi-and Nourishing-Yin Formula, general treatment and anti-infective therapy for 14 days respectively. Chest X ray or CT, WBC and its classification, ESR, CRP, hepatic function, renal function, ECG, and sputum culture were recorded and TCM symptom effects were evaluated before and after 14-day treatment. Statistics showed the curative effect, WBC, ESR, CRP and chest X ray or CT were improved in the treatment group, indicating that the method of benefiting qi and nourishing yin can improve curative effect of CAP. |
